厚朴三物汤加味方对腹股沟无张力疝修补术后患者胃肠运动功能的影响

Effect of Modified Recipe of Houpu Sanwu Decoction on Gastrointestinal Motor Function after Transabdominal Preperitoneal Herniorrhaphy

目的:观察腹腔镜腹股沟疝无张力修补术(TAPP)术后应用厚朴三物汤加味方对患者胃肠运动功能的影响。方法:选取2017年8月~2019年10月期间于本院择期进行TAPP的98例腹股沟疝患者,采用简单随机抽样法分为观察组(n=49例)与对照组(n=49例),对照组术后予以常规治疗,观察组术后24h予以厚朴三物汤加味方联合治疗,比较两组治疗效果。结果:观察组肠鸣音恢复时间、排气排便时间、住院时间均明显短于对照组(P<0.05),住院费用明显少于对照组(P<0.05);观察组术后1d、2d、3d血清胃泌素含量高于对照组(P<0.05);观察组术后1d、3d肿瘤坏死因子-α(TNF-α),C反应蛋白(CRP),白细胞介素-6(IL-6)水平均低于对照组(P<0.05);观察组术后并发症发生率4.08%低于对照组(P<0.05)。结论:TAPP术后应用厚朴三物汤加味方效果良好,可有效改善患者胃肠运动功能。

Objective: To observe the effect of Houpu Sanwu Decoction on gastrointestinal motor function after laparoscopic transabdominal preperitoneal herniorrhaphy(TAPP). Methods: A total of 98 patients with inguinal hernia who underwent elective TAPP in the hospital from August 2017 to October 2019 were enrolled as the research objects. They were divided into an observation group(n=49) and a controlled group(n=49) by simple random sampling method. After surgery, the controlled group was given routine treatment, while the observation group was additionally given Houpu Sanwu Decoction at 24 h after surgery. The curative effect was compared between the two groups. Results: The recovery time of bowel sounds, time of exhaust and defecation, and hospitalization time in observation group were significantly shorter than those in the controlled group(P < 0.05), and hospitalization cost was significantly lower than that in the controlled group(P < 0.05). At 1 d, 2 d and 3 d after surgery, content of serum gastrin in the observation group was higher than that in the controlled group(P < 0.05). At 1 d and 3 d after surgery, levels of tumor necrosis factor-α(TNF-α), C-reactive protein(CRP) and interleukin-6(IL-6) in the observation group were lower than those in the controlled group(P < 0.05). The incidence of postoperative complications in the observation group was 4.08%, was lower than that in the controlled group(P < 0.05). Conclusion: The application effect of Houpu Sanwu Decoction is good after TAPP, which can effectively improve gastrointestinal motor function of patients.
