改良电动起立床训练联合电针治疗对高位脊髓损伤患者体位性低血压的影响

Effect of Modified Tilting Table Excercise plus Electro-acupuncture on Postural Hypotension for Patients with High Spinal Cord Injury

目的:观察改良电动起立床训练联合电针治疗高位脊髓损伤患者体位性低血压的临床疗效。方法:选取符合纳入标准的患者40例随机分为2组,对照组采用常规电动起立床训练加电针治疗,观察组采用改良电动起立床训练加电针治疗。结果:两组治疗10天后患者首次发生体位性低血压角度均较治疗前提高,且观察组提高程度优于对照组。观察组患者到达直立位所经过的时间少于对照组。首次进行60°站立训练时站立3min后与平卧位比较血压差、心率变化幅度相比,观察组小于对照组。结论:改良电动起立床训练联合电针治疗高位脊髓损伤患者体位性低血压缩短了患者从平卧位到直立位的治疗时间,降低了血压差及心率变化幅度,初步取得较好的临床疗效。

Objective: To investigate the effect of modified tilting table excercise plus electro-acupuncture on postural hypotension for patients with high spinal cord injury. Methods: A total of 40 patients met the criteria were randomly divided into 2 groups, 20 patients in each group. The controlled group was treated with tilting table excercise plus electro-acupuncture, while the observation group applied with modified tilting table excercise plus electro-acupuncture. The blood pressure, heart rates, clinical symptoms and the angles of standing position were recorded before and after treatment, standing up for 3 minutes. The SPSS 19.0 statistical system was used for analysis. Results: After 10 days of treatment, the angles of the first postural hypotension in both groups were higher than before, and the observation group was greater than the controlled group. Patients in the observation group took less time to reach the upright position than those in the controlled group. In the first 60°standing up training, the difference of the blood pressure and the heart rate after standing for 3 minitues was smaller in the observation group than in the controlled group. Conclusion: The modified tilting table excercise plus electro-acupuncture on postural hypotension shortens time from supine position to upright position for patients with high spinal cord injury can reduce the differences of the blood pressure and the ranges of the heart rates, and initially achieve a good clinical effect.
