清胃散加减方治疗牙周炎的Meta分析

Meta-analysis of Modified Qingwei Powder in Treatment of Periodontitis

目的:系统评价清胃散加减方治疗牙周炎的临床疗效。方法:计算机检索PubMed、Cochrane library、Embase、国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)、中文科技期刊数据库(CCD)数据库,检索清胃散加减方治疗牙周炎的随机对照试验(RCT),检索时限从建库到2020年5月。由2名研究者按照纳入和排除标准进行文献筛选及资料提取,按照Cochrane质量评分标准对纳入文献进行质量评价,运用RevMan 5.3软件进行Meta分析。结果:共纳入12个RCT,962例受试者,对照组491例,观察组471例。结果显示利用清胃散加减方观察组与对照组比较,牙周炎患者的牙周探诊深度(PD)(MD=-0.87,95%CI为-1.11~-0.62,P<0.000 01)、菌斑指数(PI)(MD=-0.60,95%CI为-0.80~-0.40),P<0.000 01、牙龈指数(GI)(MD=-0.90,95%CI为-1.06~-0.73),P<0.000 01、牙齿松动度(MD)(MD=-0.25,95%CI为-0.41~-0.08,P=0.003)、龈沟出血指数(SBI)(MD=-0.79,95%CI为-0.99~-0.58,P<0.000 01)均显著降低。结论:利用清胃散加减方治疗牙周炎临床疗效显著,且优于对照组。但纳入的临床研究在数量及质量上存在一定局限性,仍需更多大样本、多中心、高质量的RCT来进一步验证。

Objective:To systemically evaluate the clinical efficacy of modified Qingwei Powder in treatment of periodontitis.Methods:All of randomized controlled trials(RCT)about the modified Qingwei powder in treatment of periodontitis were retrieved in PubMed,Cochrane Library,Embase,CNKI,CSPD,CCD from database inception to May 2020.A total of 2 researchers conducted literature screening and data extraction according to inclusion and exclusion criteria.According to the Cochrane quality scoring standard,the quality of the included literature was evaluated,and the RevMan 5.3 software was used for meta-analysis.Results:A total of 12 trials with 962 patients were include,with 491 patients in the control group and 471 patients in the test group.The results showed that compared with the control group,modified Qingwei Powder Formula group can significantly decrease the pocket probing depth[MD=-0.87,95%CI(-1.11 to-0.62),P<0.00001],plaque index[MD=-0.60,95%CI(-0.80 to-0.40),P<0.00001],gingival index[MD=-0.90,95%CI(-1.06 to-0.73),P<0.00001],tooth mobility degree[MD=-0.25,95%CI(-0.41 to-0.08),P=0.003]and sulcus bleeding index[MD=-0.79,95%CI(-0.99 to-0.58),P<0.00001].Conclusion:Modified Qingwei Powder in the treatment of periodontitis is significant and superior to the control group.But in view of the limitations in the quantity and quality of clinical studies,this conclusion still needs to be further verified by more large-scale,multicentric and high-quality randomized controlled trials.
