针刺联合艾灸督脉组穴治疗急性缺血性卒中的临床研究

Clinical Research of Scalp Acupuncture Combined with Moxibustion on the Points of the Governor Vessel Group for Acute Ischemic Stroke

目的观察头针排刺联合艾灸督脉组穴治疗急性缺血性卒中的效果,指导急性缺血性卒中治疗和预后现状的改善。方法按随机数字表法将88例急性缺血性卒中患者分为两组各44例,在常规治疗基础上,观察组给予头针排刺联合艾灸督脉组穴治疗,而对照组仅给予艾灸督脉组穴治疗。治疗前及治疗4周后,评估并比较两组的中医证候评分、精神状态[采用简易精神状态检查法(MMSE)评价]、认知功能[采用蒙特利尔认知评估量表(MoCA)评估]、运动功能[通过Fugl-Meyer(FMA)评分评估]、日常生活功能[通过Barthel(BI)指数评分评估]。结果治疗后,两组神识昏蒙、偏身瘫痪、感觉异常、言语不利、口舌喎斜评分均较治疗前降低,且观察组低于对照组(P<0.05),两组MMSE评分、MoCA评分均较治疗前升高,且观察组高于对照组(P<0.05);两组FMA评分、BI指数均较治疗前升高,且观察组高于对照组(均P<0.05)。结论头针排刺联合艾灸督脉组穴治疗急性缺血性卒中可帮助患者临床症状和精神状态更好更快改善,更利于促进患者认知功能、运动功能及日常生活功能的恢复。

Objective:To observe the effect of scalp acupuncture combined with moxibustion on the points of the Governor Vessel group for acute ischemic stroke,to guide the improvement of treatment of acute ischemic stroke and the condition of prognosis.Methods:According to the random number table method,88 patients with acute ischemic stroke were divided into two groups,with 44 cases in each group.On the basis of conventional treat⁃ment,the observation group was given scalp acupuncture combined with moxibustion on the points of the Governor Vessel group,while the control group was given only moxibustion on the points of the Governor Vessel group.Be⁃fore treatment and at 4 weeks of treatment,the TCM syndrome scores,mental state[using Mini-mental state exami⁃nation(MMSE)for evaluation],and cognitive function[using the Montreal Cognitive Assessment Scale for assess⁃ment],motor function[evaluated by Fugl-Meyer(FMA)score],daily life function[evaluated by Barthel(BI)in⁃dex score]of the two groups were evaluated and compared.Results:After treatment,the scores of dizziness,hemi⁃plegia,paresthesia,speech impairment/inability,and oblique oropharyngeal oblique scores in the two groups were lower than those before treatment,and the observation group was lower than the control group;the MMSE score and MoCA score of the two groups were higher than those before treatment,and the observation group was higher than the control group;the FMA score and BI index of the two groups were higher than those before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Con⁃clusion:The treatment of acute ischemic stroke with scalp acupuncture combined with moxibustion on the Gover⁃nor Vessel group points can help patients improve their clinical symptoms and mental state better and faster,and is more conducive to promoting the recovery of patients′cognitive function,motor function and daily life function.
