清肺止咳平喘汤治疗慢性阻塞性肺疾病急性加重期(痰热壅肺证)的疗效观察

Efficacy observation of Qingfei Zhike Pingchuan Decoction in the Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease(Syndrome of Phlegm-heat Congesting Lung)

目的观察清肺止咳平喘汤治疗慢性阻塞性肺疾病急性加重期(痰热壅肺证)的疗效。方法患者126例依据随机对照法分为治疗组与对照组各63例。对照组严格按照指南制定治疗方案治疗。治疗组在对照组基础上口服清肺止咳平喘汤治疗。两组均治疗10 d。比较两组临床疗效、中医证候积分、肺功能指标、血清白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、血清淀粉样蛋白A(SAA)水平。结果治疗组治疗总有效率为90.48%,高于对照组的76.19%(P<0.05);两组治疗后中医证候积分较治疗前降低,治疗组降低幅度优于对照组(P<0.05);两组治疗后肺活量(FVC)、第1秒用力呼气容积(FEV1)/FVC,FEV1占预计值百分比(FEV1%)均较治疗前升高(P<0.05),且治疗组升高幅度高于对照组(P<0.05);两组治疗后炎症因子SAA、IL-8、TNF-α较治疗前降低(P<0.05),治疗组降低幅度优于对照组(P<0.05)。结论清肺止咳平喘汤治疗慢性阻塞性肺疾病急性加重期(痰热壅肺证)疗效确切,可有效降低患者的中医证候积分,改善通气功能,降低炎症因子SAA、IL-8、TNF-α水平,提升临床疗效。

Objective:To observe the efficacy of Qingfei Zhike Pingchuan Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(syndrome of phlegm-heat congesting lung).Methods:A to⁃tal of 126 patients were divided into a treatment group and a control group according to randomized control meth⁃od,63 cases in each group.The control group strictly followed the GOLD guidelines to formulate a treatment plan.On the basis of the control group,the treatment group was treated with Qingfei Zhike Pingchuan Decoction orally.Both groups were treated for 10 days.The therapeutic effects,TCM syndrome scores,lung function indicators,se⁃rum interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),and serum amyloid A(SAA)levels were compared be⁃tween the two groups.Results:The total effective rate of the treatment group was higher than that of the control group(P<0.05);the scores of TCM symptoms in the two groups were lower than before treatment,and the reduc⁃tion in the treatment group was better than that in the control group(P<0.05);the FVC,FEV1/FVC,FEV1%of the two groups after treatment were higher than those before treatment,and the increase in the treatment group was higher than in the control group(P<0.05);after treatment Inflammatory factors SAA,IL-8,and TNF-ɑin the two groups were lower than before treatment,and the reduction in the treatment group was better than that in the con⁃trol group(P<0.05).Conclusion:Qingfei Zhike Pingchuan Decoction is effective in treating acute exacerbation of chronic obstructive pulmonary disease(syndrome of phlegm-heat congesting lung),which can effectively reduce the patient′s TCM syndrome score,improve ventilation,and reduce the levels of inflammatory factors such as SAA,IL-8,and TNF-ɑ,thus enhancing the clinical efficacy.
