前房穿刺在青光眼治疗中的应用价值

Application Value of Anterior Chamber Puncture in Glaucoma Treatment

目的探讨前房穿刺在青光眼治疗中的应用效果。方法将朝阳市中心医院收治的118例青光眼患者纳入研究对象,时间为2017年4月至2019年3月,随机数字表法分为试验组(n=59)和对照组组(n=59),对照组在小梁切除术前采用药物处理,试验组在小梁切除术前采用前房穿刺,对比两组患者眼压和前房深度改善情况、临床治疗效果以及并发症发生情况。结果治疗前,两组患者的眼压和前房深度比较无差异(P>0.05);治疗后6个月,两组患者的眼压和前房深度均改善,且试验组的改善幅度优于对照组(P<0.05);试验组并发症发生率明显低于对照组(6.77%vs.27.11%)(P<0.05);试验组临床治疗总有效率显著高于对照组(96.61%vs.77.97%)(P<0.05)。结论在为青光眼患者实施小梁切除手术之前,为其施以前房穿刺处理比采用药物处理的治疗效果更佳,在一定程度上保证了手术的成功性和安全性,有助于眼压指标的改善,并可减少术后并发症的发生。

Objective To discuss the application effect of anterior chamber puncture in glaucoma treatment.Methods A total of 118 glaucoma patients admitted to our hospital from April 2017 to March 2019 were randomly selected by digital method and divided into experimental group and control group,59 cases in each group.The control group was treated with drugs before trabeculectomy,and the experimental group was treated with anterior chamber puncture before trabeculectomy.The improvement of intraocular pressure and anterior chamber depth index,clinical treatment effect and complication rate of the two groups were compared.Results Before treatment,there was no significant change in IOP and anterior chamber depth between the two groups(P>0.05).Six months after the operation,the improvement degree of intraocular pressure and anterior chamber depth index of the two groups were improved,and the improvement degree of the experimental group was better than that of the control group,the difference was statistically significant(P<0.05).The incidence of complications in the experimental group was significantly lower than that in the control group(6.77%vs.27.11%)(P<0.05).The total effective rate of clinical treatment in the experimental group was significantly higher than that in the control group(96.61%vs.77.97%).(P<0.05).Conclusion Anterior atrial puncture is more effective than drug treatment before trabeculectomy for glaucoma patients.To a certain extent,it ensures the success and safety of the surgery,contributes to the improvement of intraocular pressure index and the occurrence of postoperative complications.
