复合式小梁切除术和单纯小梁切除术治疗青光眼患者的临床疗效

Clinical Efficacy of Combined Trabeculectomy and Simple Trabeculectomy in the Treatment of Glaucoma

目的比较复合式小梁切除术和单纯小梁切除术治疗青光眼患者的临床疗效。方法选取2019年1月至2019年7月前来本院就诊的80例青光眼患者作为调研对象,随机进行分组,每组40例。对照组患者采用单纯小梁切除术治疗,观察组采用复合式小梁切除术治疗,比较两组围手术期治疗效果。结果观察组患者在术后半年的眼压控制在12 mm Hg以下的比例为92.50%(37/40),对照组为72.50%(29/40),差异有统计学意义(P<0.05)。观察组患者成功性滤过泡成功率为90.00%(36/40),对照组为67.50%(27/40),差异有统计学意义(P<0.05)。两组患者在术后1 d即可形成前房,在术后4~14 d可调节缝线,并根据眼压状况、过滤泡进程拆除。对照组患者中有11例患者出现了浅前房,观察组患者中则有2例。观察组患者的满意度为92.50%(37/40),明显高于对照组的75.00%(30/40),差异有统计学意义(P<0.05)。观察组患者的并发症发生率为20.00%,对照组为7.50%,差异有统计学意义(P<0.05)。结论采用复合式小梁切除术治疗青光眼能够更好的改善患者的病情,有利于减少术后浅前房的发生,并能够良好的控制远期眼压状况,有利于功能性滤过泡的形成,从而保证手术的成功率,受到了患者的肯定。

Objective To compare the clinical efficacy of compound trabeculectomy and simple trabeculectomy in the treatment of glaucoma patients.Methods A total of 80 glaucoma patients who came to our hospital from January 2019 to July 2019 were selected as the survey subjects and randomly divided into groups,with 40 cases in each group.Patients in the control group were treated with simple trabeculectomy,and the observation group was treated with compound trabeculectomy.The perioperative treatment effects of the two groups were compared.Results The proportion of patients who could control the IOP below 12 mm Hg in half a year after operation was 92.50%(37/40),compared with 72.50%(29/40)in the control group,the difference was statistically significant(P<0.05).The success rate of successful filtering blebs in the observation group was 90.00%(36/40)and that in the control group was 67.50%(27/40),the difference was statistically significant(P<0.05).The anterior chamber can be formed in the two groups of patients 1 day after surgery,and the sutures can be adjusted 4-14 days after surgery,and they can be removed according to intraocular pressure and the progress of the filter bubble.Shallow anterior chamber occurred in 11 patients in the control group and 2 in the observation group.The satisfaction degree of patients in the observation group was 92.50%(37/40),which was significantly higher than 75.00%(30/40)in the control group,and the difference was statistically significant(P<0.05).The incidence of complications was 20.00%in the observation group and 7.50%in the control group,the difference was statistically significant(P<0.05).Conclusion Compound trabeculectomy can better improve the condition of glaucoma,reduce the occurrence of shallow anterior chamber,control the longterm intraocular pressure,and facilitate the formation of functional filtering blebs,so as to ensure the success rate of surgery,which has been affirmed by patients.
