断指再植术治疗拇指近节完全离断患者的临床效果和安全性

Clinical Efficacy and Safety of Replantation of Severed Fingers in the Treatment of Complete Amputation of Proximal Segment of Thumb

目的观察断指再植术在拇指近节完全断离患者治疗过程中的临床效果与安全性。方法选取2018年2月至2019年2月在我院接受拇指近节完全断离手术治疗的55例患者,所有患者的手术方法均为断指再植术,观察患者治疗后患指功能恢复情况、并发症发生情况,并对患者治疗前后的手指活动度、手指握力以及焦虑、抑郁程度进行对比。结果55例患者治疗后患指功能恢复优良率为98.18%;并发症发生率为7.27%;治疗后手指活动度、手指握力以及SAS、SDS评分与治疗前相比,差异均有统计学意义(P<0.05)。结论断指再植术可促进拇指近节完全断离患指功能的更好恢复,减少并发症的出现,提高患者治疗后手指活动度、手指握力,改善患者的焦虑、抑郁情绪,可将其作为拇指近节完全断离患者的理想手术方案。

Objective To observe the clinical efficacy and safety of replantation of severed fingers in the treatment of patients with complete amputation of proximal segment of thumb.Methods The subjects of this experimental study were 55 patients who underwent complete amputation of the proximal segment of thumb in our hospital from February 2018 to February 2019.All patients underwent replantation of severed fingers.The recovery of finger function and incidence of complications after treatment were observed and the finger mobility,finger grip strength,anxiety and depression scores before and after treatment were compared and analyzed.Results The excellent and good recovery rate of finger function after treatment was 98.18%in 55 patients.The incidence of complications was 7.27%.After treatment,finger movement,finger grip strength,anxiety and depression scores were significantly different from those before treatment(P<0.05).Conclusion The application of amputated finger replantation can promote better recovery of the function of the proximal segment of the thumb completely severed from the affected finger,reduce the occurrence of complications,improve the patient’s finger mobility and finger grip after treatment,and improve the patient’s anxiety and depression.It is an ideal surgical plan for patients with complete severation of the proximal part of the thumb.
