老年急性心力衰竭代偿期病人早期简易心脏康复方案的构建及应用

Construction and application of early simple cardiac rehabilitation program for elderly patients with acute heart failure in compensatory period

目的构建老年急性心力衰竭代偿期病人早期简易心脏康复方案并探讨其干预效果。方法成立运动康复小组,文献检索后形成早期简易心脏康复初步方案,经专家小组会议探讨后形成最终方案。采用任意抽样法选取2015年8月至2020年8月于我院住院治疗的老年急性心力衰竭代偿期病人104例作为研究对象,观察组和对照组各52例。对照组进行常规运动宣教,观察组采用四阶梯式早期简易心脏康复。记录并比较2组干预前、后相关指标的差异。结果干预后,观察组明尼苏达生活质量量表评分、N-末端B型利钠肽原(NT-proBNP)水平显著低于对照组(P<0.05或0.01);FEV1、6 min步行试验距离、30 s手臂屈曲次数显著高于对照组(P<0.05或0.01)。结论早期简易心脏康复方案可通过团队监督管理保障病人早期运动安全顺利进行,并可改善老年急性心力衰竭代偿期病人心功能,提高生活质量。

Objective To construct an early simple cardiac rehabilitation program for the elderly patients with acute heart failure in the compensatory phase and to explore the intervention effects.Methods A sports rehabilitation team was established,and the preliminary plan for early simple cardiac rehabilitation was formed after literature search,and the final plan was formed after the discussion of the expert group meeting.An arbitrary sampling method was used to select 104 elderly patients with compensatory acute heart failure who were hospitalized in our hospital from August 2015 to August 2020 as the research objects,with 52 cases in the observation group and the control group.The related indicators before and after the intervention of the early simple cardiac rehabilitation program in the two groups were recorded and analyzed.Results The Minnesota Living with Heart Failure Question maire score and the level of amino-terminal pro-brain natriuretic peptide in the observation group were lower than those in the control group(P<0.05 or 0.01);The levels of the forced expiratory volume in one second(FEV1),6 minutes walk distance and the times of 30-second arm curl were significantly higher than those in the control group(P<0.05 or 0.01).Conclusions The early simple cardiac rehabilitation program can ensure the safe progress of the patients’early exercise through team supervision and management,and can improve the cardiac function of the elderly patients with acute heart failure in the compensatory phase and improve the quality of life.
