2型糖尿病患者实施达格列净联合胰岛素治疗的效果分析

Analysis of the Effect of Dapagliflozin Combined with Insulin Therapy in Type 2 Diabetic Patients

目的探讨2型糖尿病患者实施达格列净联合胰岛素治疗的效果。方法选取2018年1月—2019年10月118例2型糖尿病患者为研究对象,随机数字表法分为观察组和对照组,每组59例。对照组采取门冬胰岛素30治疗,观察组在对照组基础上采取达格列净治疗,观察两组患者临床治疗效果、不良事件发生情况与糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后血糖(PPG)、胰岛素日总量(TDDI)。结果观察组治疗总有效率98.31%高于对照组86.44%,差异有统计学意义(χ^(2)=5.894,P=0.015);治疗后,观察组HbA1c(7.68±0.54)%、TDDI(40.69±2.21)U/d、FPG(6.95±0.36)mmol/L、PPG (8.68±0.85)mmol/L均低于对照组(7.95±0.38)%、(56.98±3.85)U/d、(7.85±0.41)mmol/L、(9.75±0.73)mmol/L,差异有统计学意义(t=3.141、28.187、12.670、7.335,P<0.05);观察组患者低血糖发生率1.69%明显低于对照组11.86%,差异有统计学意义(χ^(2)=4.827,P=0.028)。结论 2型糖尿病患者实施达格列净联合胰岛素治疗的效果显著,可降低胰岛素日总量,预防低血压,应用价值高。

Objective To investigate the effect of dapagliflozin combined with insulin therapy in patients with type 2 diabetes.Methods From January 2018 to October 2019,118 patients with type 2 diabetes were selected as the research objects,and they were divided into observation group and control group by random number table,with 59 cases in each group.The control group was treated with insulin aspart 30,and the observation group was treated with dapagliflozin on the basis of the control group.The clinical treatment effects,adverse events,and glycosylated hemoglobin(HbA1c),fasting blood glucose(FPG),and postprandial blood glucose(PPG),total daily insulin(TDDI)were observed in the two groups.Results The total effective rate of treatment in the observation group was 98.31%higher than that in the control group 86.44%,and the difference was statistically significant(χ^(2)=5.894,P=0.015);after treatment,the observation group had HbA1c(7.68±0.54)%and TDDI(40.69±2.21)U/d,FPG(6.95±0.36)mmol/L,PPG(8.68±0.85)mmol/L are all lower than the control group(7.95±0.38)%,(56.98±3.85)U/d,(7.85±0.41)mmol/L,(9.75±0.73)mmol/L,the difference was statistically significant(t=3.141,28.187,12.670,7.335,P<0.05);the incidence of hypoglycemia in the observation group was 1.69%significantly lower than that of the control group.The difference was statistically significant(χ^(2)=4.827,P=0.028).Conclusion The effect of dapagliflozin combined with insulin therapy in type 2 diabetic patients is significant,which can reduce the daily total insulin,prevent hypotension,and has high application value.
