单双倍循环血量换血治疗新生儿高胆红素血症的对照分析

摘要 目的对新生儿高胆红素血症采用单双倍循环血量换血治疗的效果进行探究。方法选择2017年1月—2019年12月该院收治的新生儿高胆红素血症患儿80例作为研究对象,按照随机数表法将其划分为单倍组和双倍组,每组40例。采用双倍循环血量换血对双倍组予以治疗,采用单倍循环血量换血对单倍组予以治疗,对比两组患儿的治疗效果。结果单倍组的血小板计数、血钠、血糖内环境指标(140.5±9.1)×10^(9)/L、(129.8±8.4)mmol/L、(9.7±1.4)mmol/L变化幅度较之于双倍组(120.6±9.2)×10^(9)/L、(110.4±6.4) mmol/L、(7.2±1.1)mmol/L明显更低,差异有统计学意义(t=9.726、11.618、8.881,P<0.05);单倍组与双倍组治疗期间的不良反应发生率(2.5%vs 5.0%)差异无统计学意义(χ^(2)=0.157,P>0.05)。结论单倍循环血量换血治疗新生儿高胆红素血症能够有效减少其血胆红素,而且对患儿的内环境影响较小,不良反应较少,具有较高的安全性。 Objective To explore the effect of single and double circulating blood transfusion therapy for neonatal hyperbilirubinemia.Methods 80 neonates with hyperbilirubinemia in the hospital from January 2017 to December 2019 were selected as the research objects included in this study,and they were divided according to the random number table method:single group and double group,each group had 40 cases.Double circulating blood volume exchange transfusion was used to treat the double group,and the single circulating blood volume exchange transfusion was used to treat the single group.The treatment effects of the two groups of children were compared.Results The platelet count,blood sodium,and blood glucose internal environmental indicators of the Single group(140.5±9.1)×10^(9)/L,(129.8±8.4)mmol/L,(9.7±1.4)mmol/L changed range compared with the double group(120.6±9.2)×10^(9)/L,(110.4±6.4)mmol/L,(7.2±1.1)mmol/L was significantly lower,the difference was statistically significant(t=9.726,11.618,8.881,P<0.05);there was no statistically significant difference in the incidence of adverse reactions(2.5%vs 5.0%)in the single group and double group during treatment(χ^(2)=0.157,P>0.05).Conclusion Single-fold-circulating blood exchange transfusion can effectively reduce the blood bilirubin of neonates with hyperbilirubinemia,and has less impact on the internal environment of the children,with fewer adverse reactions and high safety.
