心房纤颤应用心电图检查诊断的准确率及漏诊率临床探讨

Clinical Study on the Accuracy and Missed Diagnosis Rate of Atrial Fibrillation by Electrocardiogram

目的分析心房纤颤(简称AF)应用心电图检查诊断的准确率及漏诊率。方法选取2018年3月—2020年3月间来院治疗的88例AF患者,对其进行心电图检查,评估其检查效果。结果所有患者中,细波型发生率为38.64%,粗波型发生率为61.36%。所有患者的较慢型AF发生率为55.68%,快速型发生率为30.68%,极速型发生率为13.64%。肺心病(简称PHD)的脑梗死发生率为1.14%,心力衰竭发生率为5.68%;风湿性心脏病(简称RHD)的脑梗死发生率为1.14%,心力衰竭发生率为4.55%;冠心病(简称CHD)的脑梗死发生率为6.82%,心力衰竭发生率为12.50%;甲状腺功能亢进性心脏病(简称THD)的脑梗死发生率为0.00%,心力衰竭发生率为2.27%;高血压心脏病(简称HHD)的脑梗死发生率为4.55%,心力衰竭发生率为7.95%。所有患者合并脑梗死的发生率为13.64%,合并尽力衰竭的发生率为32.95%。心电图检查的准确率为96.59%,漏诊率为2.38%,敏感度为97.62%,特异度为75.00%。结论心电图检查可有效检出AF类型,且能分析其高发合并症,具有较高的诊断准确率,漏诊率低,可科学指导临床治疗方案。

Objective To analyze the accuracy and missed diagnosis rate of atrial fibrillation(AF)by electrocardiography.Methods The subjects of this study were 88 patients with AF who came to the hospital for treatment from March 2018 to March 2020.Perform an electrocardiogram examination to evaluate the effect of the examination.Results Among all patients,the incidence of fine-wave mode was 38.64%,and the incidence of coarse-wave mode was 61.36%.The incidence of slower type AF was 55.68%,the rapid type was 30.68%,and the extreme type was 13.64%in all patients.The incidence of cerebral infarction in pulmonary heart disease(PHD)was 1.14%,and the incidence of heart failure was 5.68%;the incidence of cerebral infarction in rheumatic heart disease(RHD)was 1.14%,and the heart failure rate was 4.55%;the incidence of cerebral infarction in coronary heart disease(CHD)was 6.82%,and the incidence of heart failure was 12.50%;thyroid function Hypertensive heart disease(THD)had a cerebral infarction rate of 0.00%,and heart failure had a rate of 2.27%;hypertensive heart disease(HHD)has a cerebral infarction rate of 4.55%.The incidence of heart failure was 7.95%.The incidence of cerebral infarction in all patients was 13.64%,and the incidence of heart failure was 32.95%.The accuracy rate of ECG examination was 96.59%,missed diagnosis rate was 2.38%,sensitivity was 97.62%,specificity was 75.00%.Conclusion ECG can effectively detect the type of AF,and can analyze its high incidence of comorbidities.It has a high diagnostic accuracy rate and a low missed diagnosis rate.It can scientifically guide the clinical treatment plan.
