急性压疮护理干预对长时间侧卧位手术患者压疮形成及不良反应的影响

Effect of nursing intervention of acute pressure ulcer on pressure ulcer formation and adverse reactions in patients undergoing long-term lateral surgery

目的:观察急性压疮护理干预对长时间侧卧位手术患者压疮形成及不良反应的影响。方法:选取2018年2月~2020年3月108例长时间侧卧手术患者分为干预组和对照组,对照组给予常规护理,干预组在手术过程中给予急性压疮护理干预,比较两组患者不同时间点压疮发生情况、压疮面积,不良反应发生情况,术后拔管时间及凝血功能水平,护理满意度评价。结果:干预组患者术后即刻、术后0.5 h及术后24 h压疮发生率较对照组患者均显著下降、压疮面积较对照组患者均显著缩小(P<0.05),术后干预组患者肢体麻木、紧张情绪、腰肌疼痛、颈肩痛发生率较对照组患者均显著下降(P<0.05),干预组患者拔管时间、凝血酶原时间及部分活化凝血酶时间较对照组患者均显著缩短(P<0.05),对护理满意度评分(质量、责任心、态度)较对照组患者均显著提高(P<0.05)。结论:长时间侧卧手术的患者术中采取急性压疮护理干预是有效的,可以减少压疮的形成,降低不良反应发生率,提高患者对于护理服务的满意度。

Objective:To investigate the effect of acute pressure ulcer nursing intervention on pressure ulcer formation and adverse reactions in patients with long-term lateral surgery.Method:108 patients with long-term lateral surgery from February 2018 to March 2020were selected and divided into intervention group and the control group.And the control group were given routine nursing care.The intervention group was given acute pressure ulcer nursing intervention during the operation.The occurrence of pressure ulcers at different time points,the area of pressure ulcers,the occurrence of adverse reactions,postoperative extubation time and blood clotting,evaluation of function and nursing satisfaction were compared between the two groups.Results:The incidence of pressure ulcers in the intervention group immediately after surgery,0.5h after surgery and 24h after surgery were significantly lower than those in the control group,and the area of pressure ulcers was significantly smaller than that in the control group(P<0.05),the incidence of limb numbness,nervousness,lumbar muscle pain,neck and shoulder pain in the intervention group were significantly lower than those in the control group(P<0.05).The extubation time,prothrombin time and partially activated coagulation in the intervention group Enzyme time were significantly shorter than those of the control group(P<0.05),and the scores of nursing satisfaction(quality,responsibility,attitude)were significantly higher than those of the control group(P<0.05).Conclusion:Intraoperative care intervention for patients with long-term lateral surgery for acute pressure ulcers is effective,which can reduce the formation of pressure ulcers,reduce the incidence of adverse reactions,and improve patients'satisfaction with nursing services.
