多层螺旋CT在肺混合磨玻璃结节早期诊断中的应用

摘要 目的研究多层螺旋CT在肺磨玻璃结节(Ground-Glass Nodule,GGN)早期诊断中的应用,以期为肺癌的早期诊断提供临床参考。方法回顾性分析2018年10月至2020年2月,在陕西省康复医院的多层螺旋CT仪引导下进行穿刺活检或手术后病理学检查确诊的GGN患者84例,研究其CT检查影像学资料,对比分析其良性患者、无典型症状患者及恶性患者之间是否存在差异。结果84例患者中,有恶性病变患者53例、良性病变患者22例和无典型症状患者9例,3组患者分别在病灶大小、形状、边界清晰度、边缘形态等方面存在显著差异,具有统计学意义(P<0.05)。对比良性GGN患者、无典型症状患者、恶性GGN患者的年龄及病灶大小,年龄:良性GGN患者<无典型症状患者<恶性GGN患者,病灶大小:恶性GGN患者>良性GGN患者>无典型症状患者(P<0.05)。单纯GGN患者多为良性(81.82%),其病灶形状多为不规则形状、边缘形态多为毛刺、磨玻璃结节边界多为模糊。恶性GGN患者的病灶边缘多为分叶且比例显著高于良性患者(P<0.05)。结论不同类型的GGN患者的多层螺旋CT影像表现不同,多层螺旋CT对GGN患者的良恶性鉴别具有较高的识别率。 Objective To study the application of multi-slice spiral CT(MSCT)in the early diagnosis of pulmonary mixed ground-glass nodule(GGN),in order to provide clinical reference for the early diagnosis of lung cancer.Methods 84 patients from October 2018 to February 2020 with GGN who were diagnosed by biopsy or operation and pathology under the guidance of MSCT in our hospital were analyzed retrospectively.The imaging data of CT examination were studied and the differences among benign patients,atypical patients and malignant patients were compared and analyzed.Results Among the 84 patients,there were 53 patients with malignant lesions,22 patients with benign and 9 patients without typical symptoms.There were significant differences in the size,shape,boundary definition and edge morphology of the three patients(P<0.05).The Results showed that the age of patients with benign GGN was lower than that of patients without typical symptoms and that of patients with malignant GGN was higher than that of patients with benign GGN(P<0.05).Most of the patients with simple GGN were benign(81.82%),the shape of the lesions was irregular,the shape of the edge was burr,and the boundary of the ground glass nodules was fuzzy.The margin of lesions in patients with malignant GGN was mostly lobulated,and the proportion was significantly higher than that in patients with benign GGN(P<0.05).Conclusion Different types of GGN patients have different MSCT images.MSCT has a high recognition rate for the differentiation of benign and malignant GGN patients,which is worthy of clinical application.
