摘要 目的探讨呼吸道病原体IgM抗体检测在呼吸道感染中的应用效果。方法选择2018年8月—2020年2月该院收治1800例呼吸道感染患者作为研究对象。所有患者均以间接荧光免疫法对8种呼吸道病原体IgM抗体进行检测,包括甲型流感病毒(InfluenzaA virus,IFA)、乙型流感病毒(InfluenzaB virus,IFB)、肺炎支原体(Mycoplasma pneumoniae,Mp)、呼吸道合胞病毒(Respiratory syncytialvirus,RSV)、嗜肺军团菌(Legionella pneumophila,LP)、腺病毒(Adenovirus,ADV)、肺炎衣原体(Chlamydia pneumoniae,CP)和副流感病毒(Parainfluenza virus,PIVS)。观察患者呼吸道8种病原体IgM抗体阳性结果分布情况、呼吸道8种病原体IgM抗体多重感染阳性分布情况,并对比不同年龄段患者呼吸道8种病原体IgM抗体检测的阳性率。结果该组1800例呼吸道感染患者共有655例IgM抗体为阳性,阳性率为36.39%,其他依次为RSV11.22%、ADV6.67%、CP3.61%、IFA3.39%、IFB1.06%、PIVS0.67%、LP0.28%。该组呼吸道8种病原体IgM抗体单一感染阳性率61.83%、二重感染25.19%、三重感染11.45%、四重感染1.53%。该组年龄<18岁患者Mp IgM抗体检测的阳性率83.64%高于年龄18~59岁73.04%、≥60岁70.97%,差异有统计学意义(χ2=6.509、11.085,P<0.05)。结论Mp是引起呼吸道感染的重要病原体,采用间接免疫荧光法检测呼吸道病原体IgM抗体对于呼吸道感染患者早期诊断具有显著的意义,能够有效避免抗菌药的盲目应用。 Objective To explore the application effect of IgM antibody detection of respiratory pathogens in respiratory tract infections.Methods From August 2018 to February 2020,1800 patients with respiratory tract infections were selected as the research objects.All patients in this group were tested for IgM antibodies against 8 respiratory pathogens by indirect fluorescence immunoassay,including:Influenza A virus(IFA),Influenza B virus(IFB),Mycoplasma pneumoniae,Mp),Respiratory syncytialvirus(RSV),Legionella pneumophila(LP),Adenovirus(ADV),Chlamydia pneumoniae(CP)and Parainfluenza virus(PIVS).Observe the distribution of positive results of IgM antibodies against 8 pathogens in the respiratory tract and the positive distribution of IgM antibodies against multiple infections of 8 pathogens in the respiratory tract in this group of patients,and compare the positive rates of IgM antibody detection of 8 pathogens in the respiratory tract of patients of different ages.Results A total of 655 IgM antibodies in this group of 1800 patients with respiratory infections were positive,with a positive rate of 36.39%.The others were RSV 11.22%,ADV 6.67%,CP 3.61%,IFA 3.39%,IFB 1.06%,PIVS 0.67%,LP 0.28%.In this group,the positive rate of IgM antibodies of 8 respiratory tract pathogens was 61.83%for single infection,25.19%for double infection,11.45%for triple infection,and 1.53%for quadruple infection.The positive rate of Mp IgM antibody detection in this group of patients<18 years old was 83.64%higher than that of patients aged 18-59 years old(73.04%),≥60 years old 70.97%,the difference was statistically significant(χ2=6.509,11.085,P<0.05).Conclusion Mp is an important pathogen causing respiratory tract infections.The indirect immunofluorescence method to detect respiratory pathogen IgM antibodies has significant significance for the early diagnosis of patients with respiratory tract infections,and can effectively avoid the blind application of antibacterial drugs.
