摘要
目的分析经皮微创脊柱内固定系统治疗创伤性胸腰段骨折的效果。方法将符合纳入标准的2017年3月—2020年4月60例创伤性胸腰段骨折患者作为该次研究对象,按照入院时间分组,分别行后路椎弓根内固定(对照组)及经皮微创脊柱内固定系统治疗(观察组),比较两组的手术指标、关节功能恢复效果及炎性因子水平。结果观察组切口长度(3.65±0.48)cm,手术时间(72.16±12.76)min,出血量(65.27±7.29)mL,住院时间(5.26±1.18)d,骨折愈合时间(9.11±0.27)周,优于对照组,差异有统计学意义(t=21.609、10.014、34.496、13.822、20.744,P<0.05);观察组伤椎椎体高度比(91.32±16.28)%,Cobb角为(8.16±1.38)°,VAS评分(6.22±3.27)分,优于对照组,差异有统计学意义(t=4.003、6.691、10.961,P<0.05);观察组术后CRP是(8.26±1.28)mg/L,TNF-α为(46.26±6.28)pg/mL,IL-1β是(23.18±2.74)pg/mL,均低于对照组,差异有统计学意义(t=8.212、5.503、11.096,P<0.05)。结论经皮微创脊柱内固定系统治疗创伤性胸腰段骨折利于骨折以及关节功能的恢复,且手术创伤更小,康复速度较快,术后炎性反应轻,不易发生感染等并发症。
Objective To analyze the effect of percutaneous minimally invasive spinal internal fixation system in the treatment of traumatic thoracolumbar fractures.Methods Sixty patients with traumatic thoracolumbar fractures who met the inclusion criteria were included in this study.The patients were enrolled from March 2017 to April 2020.They were grouped according to the time of admission and received posterior pedicle internal fixation(control group)and percutaneous minimally invasive spinal internal fixation system(observation group),compared the surgical indicators,joint function recovery effect and inflammatory factor levels of the two groups.Results The length of incision in observation group was(3.65±0.48)cm,operation time(72.16±12.76)min,blood loss(65.27±7.29)mL,hospitalization time(5.26±1.18)d,fracture healing time(9.11±0.27)week,more excellent than the data of the control group,the difference was statistically significant(t=21.609,10.014,34.496,13.822,20.744,P<0.05);the height ratio of the injured vertebral body in the observation group was(91.32±16.28)%,the Cobb angle was(8.16±1.38)°,and the VAS score was(6.22±3.27)points,better than the data of the control group,the difference was statistically significant(t=4.003,6.691,10.961,P<0.05);postoperative CRP of the observation group was(8.26±1.28)mg/L,and TNF-αwas(46.26±6.28)pg/mL,IL-1βwas(23.18±2.74)pg/mL,which was lower than the control group data,the difference was statistically significant(t=8.212,5.503,11.096,P<0.05).Conclusion The percutaneous minimally invasive spinal fixation system for the treatment of traumatic thoracolumbar fractures is beneficial to the recovery of fractures and joint function,and the surgical trauma is less,the recovery speed is faster,the postoperative inflammatory reaction is light,and complications such as infection are less likely to occur.
