宫腔镜下刮匙刮除和电切除术对子宫内膜息肉的疗效差异研究

Study on the Difference of Curettage and Electric Resection Under Hysteroscope for Endometrial Polyps

目的探究宫腔镜下刮匙刮除术和电切除术对子宫内膜息肉的疗效差异。方法择取2018年3月—2019年4月期间该院收治的82例子宫内膜息肉患者参与研究,根据抽签法进行随机分组,包括对照组41例和观察组41例。对照组患者行宫腔镜下电切除术治疗,观察组患者行宫腔镜下刮匙刮除术治疗,比较两组患者手术用时、术中失血量、住院时间,治疗前后排卵期子宫内膜厚度、月经量,随访12个月统计两组患者疾病复发率和妊娠率。结果治疗后观察组患者排卵期子宫内膜厚度(10.13±1.68)mm高于对照组(7.44±1.51)mm,月经量(136.41±8.85)mL少于对照组(167.42±10.12)mL,差异有统计学意义(t=7.625、14.769,P<0.05);术后12个月观察组患者复发率(26.83%)和妊娠率(73.17%)均高于对照组4.88%、46.34%,差异有统计学意义(χ2=5.851、5.071,P<0.05)。结论宫腔镜下刮匙刮除术和电切除术对子宫内膜息肉均有一定的疗效,不同术式适合不同情况的患者,可根据患者的实际需求选择合适术式,保证患者疗效。

Objective To explore the difference of curettage and electric resection for endometrial polyps under hysteroscope.Methods A total of 82 patients with endometrial polyps admitted to the hospital from March 2018 to April 2019 were selected to participate in the study,and randomized grouping according to the lottery method,including 41 cases in the control group and 41 cases in the observation group.Patients in the control group underwent hysteroscopic electric resection,and patients in the observation group underwent hysteroscopic curettage.The operation time,intraoperative blood loss,length of stay in the two groups were compared,and the endometrial thickness during ovulation,menstrual volume before and after treatment were compared,followed up for 12 months,statistics of disease recurrence rate and pregnancy rate of the two groups of patients.Results After treatment,the endometrial thickness(10.13±1.68)mm of the observation group during ovulation was higher than that of the control group(7.44±1.51)mm,and the menstrual flow(136.41±8.85)mL was less than the control group(167.42±10.12)mL,the difference between the groups was statistically significant(t=7.625,14.769,P<0.05);the recurrence rate(26.83%)and pregnancy rate(73.17%)of the observation group were higher than those of the control group by 4.88%and 46.34%at 12 months after operation,the difference was statistically significant(χ2=5.851,5.071,P<0.05).Conclusion Hysteroscopic curettage and electric resection have a certain effect on endometrial polyps.Different surgical procedures are suitable for patients with different conditions.The appropriate surgical procedures can be selected according to the actual needs of patients to ensure the efficacy of patients.
