早期食管癌患者内镜黏膜下剥离术后癌症复发恐惧的影响因素

Influencing Factors of Fear of Cancer Recurrence in Patients with Early Esophageal Cancer after Endoscopic Submucosal Dissection

目的分析早期食管癌患者内镜黏膜下剥离(ESD)术后癌症复发恐惧的影响因素。方法选择2018年11月至2020年11月我院104例早期食管癌ESD术后患者,采用恐惧疾病进展简化量表(Fop-Q-SF)评估患者术后癌症复发恐惧。统计患者基线资料、病灶数,采用社会支持评定量表(SSRS)评估患者社会支持度,分析早期食管癌患者ESD术后癌症复发恐惧的影响因素。结果104例早期食管癌患者ESD术后Fop-Q-SF评分(34.50±4.87)分;女性患者Fop-Q-SF评分高于男性,SSRS评分<20分患者Fop-Q-SF评分高于SSRS评分≥20分患者,病灶数>1个患者Fop-Q-SF评分高于单病灶数,差异有统计学意义(P<0.05);经多元线性回归分析得到方程:Y=3.290+0.051X 1+0.051X 2+0.044X 3,结果显示,性别、SSRS评分、病灶数是早期食管癌患者ESD术后癌症复发恐惧的影响因素(P<0.05)。结论早期食管癌患者ESD术后癌症复发恐惧与性别、社会支持度、病灶数密切相关。

Objective To analyze the influencing factors of fear of cancer recurrence in patients with early esophageal cancer after endoscopic submucosal dissection(ESD).Methods 104 patients with early esophageal cancer after ESD in our hospital from November 2018 to November 2020 were selected,the postoperative fear of cancer recurrence in the patients was assessed by the Fear of Progression Questionnaire-Short Form(Fop-Q-SF).The baseline data and the number of lesions of patients were counted.Social support rating scale(SSRS)was used to evaluate the social support of the patients.Results Among 104 patients after ESD,the Fop-Q-SF score was(34.50±4.87)points;the score of Fop-Q-SF in female patients was higher than that in male patients;the score of Fop-Q-SF in patients with SSRS score<20 points was higher than that of patients with SSRS score≥20 points;the score of Fop-Q-SF in patients with lesion>1 was higher than that of single lesion,there was a statistical significant difference(P<0.05);by multiple linear regression analysis,obtained the equation Y=3.290+0.051X 1+0.051X 2+0.044X 3.The results showed that gender,SSRS score and the number of lesions were the influencing factors of fear of cancer recurrence in the patients after ESD(P<0.05).Conclusion The fear of cancer recurrence in patients with early esophageal cancer after ESD is closely related to gender,social support and the number of lesions.
