三阶梯药物止痛法联合四磨汤和穴位敷贴治疗老年癌性疼痛的临床分析

Clinical analysis of three-step drug analgesia combined with Simo decoction and acupoint application in the treatment of cancer pain in elderly patients

目的探究三阶梯药物止痛法联合四磨汤和穴位敷贴对癌性疼痛患者的临床疗效。方法选取2018年6月-2020年6月遂宁市中心医院收治的78例癌性疼痛患者,根据随机数字表法分为3组:A组(n=26)、B组(n=26)与C组(n=26)。A组患者予以常规三阶梯药物止痛法治疗,B组患者予以三阶梯药物止痛法联合四磨汤治疗,C组予以三阶梯药物止痛法联合四磨汤和穴位敷贴治疗,对比分析3组患者的镇痛评分(NRS数字分级法)、镇静评分(Ramsay评分)、生活质量(肿瘤患者生活质量评分QOL)及不良反应情况。结果A组、B组患者的NRS评分、镇静评分比较差异无统计学意义(P>0.05)。C组在治疗后患者的NRS评分、镇静评分明显优于A组(P<0.05)。A组、B组在治疗后患者的食欲、睡眠、精神状态等生活质量比较差异无统计学意义(P>0.05)。C组在接受治疗后患者的生活质量评分升高,较A组、B组患者的优势显著(P<0.05)。C组患者的不良反应发生率与B组相比差异无统计学意义(P>0.05),然C组不良反应发生率相较于A组明显下降(P<0.05)。结论三阶梯药物止痛法联合四磨汤和穴位敷贴对癌性疼痛患者的临床疗效显著,缓解疼痛,降低药物不良反应,提高生活质量,具有一定临床应用价值。

Objective To explore the clinical effects of three-step drug analgesia combined with Simo decoction and acupoint application on cancer pain in elderly patients.Methods Seventy-eight patients with cancer pain who admitted to Central Hospital of Suining City from June 2018 to June 2020 were selected and randomly divided into three groups:group A(n=26),group B(n=26),and group C(n=26).Group A was treated with conventional three-step drug analgesia,group B was treated with three-step drug analgesia combined with Simo decoction,and group C was treated with three-step drug analgesia combined with Simo decoction and acupoint application.The analgesia score(numerical rating scale,NRS),sedation score(Ramsay score),quality of life(quality of life score for cancer patients,QOL)score and incidence of adverse reactions of three groups were compared and analyzed.Results There were no significant differences in NRS score and sedation score between group A and group B(P>0.05).The NRS score and sedation score of group C after treatment were significantly better than those of group A(P<0.05).There were no significant differences in the quality of life such as appetite,sleep and mental state between group A and group B after treatment(P>0.05).The QOL scores of group C increased after treatment,which had significant advantages over groups A and B(P<0.05).There was no significant difference in the incidence of adverse reactions between group C and group B(P>0.05),but the incidence of adverse reactions in group C was significantly lower than that in group A(P<0.05).Conclusion T he three-step drug analgesia combined with Simo decoction and acupoint application has significant clinical effects on cancer pain in elderly patient,which can relieve pain,reduce adverse drug reactions and improve the quality life of patients.It has certain clinical application value.
