老年缺血性脑卒中后疲劳现状及生活质量影响因素分析

Analysis of fatigue status and influencing factors of quality of life in elderly patients with ischemic stroke

目的分析老年缺血性脑卒中患者卒中后疲劳现状,并探讨影响患者生活质量的因素。方法回顾性分析2017年9月-2020年12月在四川大学华西医院神经内科住院治疗治疗的117例老年缺血性脑卒中患者的一般临床资料,分析患者在卒中后的疲劳状态,进一步分析在不同特征下脑卒中患者的生活质量,探讨影响其生活质量的因素。结果117患者中,81例正常无疲劳,36例存在疲劳情况,其中轻度疲劳12例(33.33%)、中度疲劳17例(47.22%)、重度疲劳7例(19.44%),无极重度疲劳;Logistic回归分析发现,年龄≥75岁(OR=1.439,95%CI=1.109~1.868)、家庭人均月收入≤2000元(OR=1.390,95%CI=1.042~1.854)、农村医保(OR=1.346,95%CI=1.031~1.757)、居住地农村(OR=1.480,95%CI=1.094~2.001)、回避或屈服应对疾病(OR=1.429,95%CI=1.040~1.963)、轻、中、重度疲劳(OR=1.446,95%CI=1.069~1.956)是影响脑卒中患者生活质量的独立危险因素(P<0.05)。结论老年缺血性脑卒中患者在卒中后往往会伴随疲劳症状,且疲劳状态、年龄、家庭人均月收入、居住地与应对疾病方式均会对患者生活质量产生影响,提示临床应针对以上因素对患者进行干预。

Objective To analyze the fatigue status of elderly patients with ischemic stroke,and explore factors affecting the life quality of patients.Methods The general clinical data of 117 elderly patients with ischemic stroke admitted to the Department of Neurology,West China Hospital of Sichuan University from September 2017 to December 2020 were retrospectively analyzed.The status of post-stroke fatigue and life quality of the patients with different characteristics was analyzed to explore the factors that affect the life quality of stroke patients.Results Among the 117 patients,there were 81 cases without fatigue(69.23%)and 36 cases(30.77%)with fatigue,including 12 cases(33.33%)with mild fatigue,17 cases(47.22%)with moderate fatigue,7 cases(19.44%)with severe fatigue,and no extreme severe fatigue.Logistic regression analysis found that age≥75 years old(OR=1.439,95%CI=1.109~1.868),family monthly income≤2,000 yuan(OR=1.390,95%CI=1.042-1.854),rural medical insurance(OR=1.346,95%CI=1.031~1.757),living in rural areas(OR=1.480,95%CI=1.094-2.001),avoiding or succumbing to disease(OR=1.429,95%CI=1.040-1.963),mild,moderate and severe fatigue(OR=1.446,95%CI=1.069~1.956)were independent risk factors influencing life quality of stroke patients(P<0.05).Conclusion Elderly patients with ischemic stroke are often accompanied with fatigue symptoms after stroke.All of the fatigue status,age,family monthly income,place of residence and styles coping with the disease have an impact on the life quality of patients.It is suggested that the clinical intervention should be carried out according to the above factors.
