2017—2019年天津市医疗卫生系统控烟暗访结果分析

Analysis on secret investigation on tobacco control in medical and health system of Tianjin from 2017 to 2019

目的监测天津市无烟医疗卫生系统创建效果,查找问题,总结经验,为探索有效的监管机制提供依据。方法 2017—2019年,采取实地暗访、隐蔽拍摄、填写调查问卷的形式对天津市卫生行政和公共卫生机构、医院进行暗访。结果 2019年综合评分、卫生行政及公共卫生机构评分、医院评分分别为90.8、98.9、81.8,均高于2017、2018年的评分。2019年"无烟环境"评分(52.7)高于2017、2018年,但"吸烟状况"评分(38.1)低于2017、2018年。2019年控烟宣传材料、控烟条例、吸烟区及引导标志、禁烟标识、投诉举报电话、控烟巡查员、戒烟门诊等设置评分均较2017、2018有所提高,但医疗机构依然存在所属小卖部售卖卷烟、工作人员室内吸烟的现象。结论天津市医疗卫生系统无烟环境设置有所改善,但吸烟状况仍不容乐观,要继续完善监管机制,不断推进控烟工作。

Objective To monitor the effect of the smoke-free medical and health system in Tianjin,and to provide a basis for exploring an effective regulatory mechanism.Methods From 2017 to 2019,secret visits were conducted in health administration and public health institutions and hospitals in the forms of field visits and taking pictures and filling questionnaire in Tianjin.Results The comprehensive score,the score of health administration and public health institutions,and the score of hospitals of 2019 were 90.8,98.9,and 81.8,all of them were higher than those in 2017 and2018.The score of"smoke-free environment"in 2019(52.7)was higher than that in 2017 and 2018,but the score of"smoking status"(38.1)was lower than that in 2017 and 2018.In 2019,tobacco control publicity materials,tobacco control regulations,smoking areas and guidance signs,no-smoking signs,complaint and reporting phone numbers,tobacco control inspectors,smoking cessation clinics were all improved compared to 2017 and 2018.However,there were still some phenomena of selling cigarettes in the kiosks owned by medical institutions and smoking in the staff rooms.Conclusion The setting of smoke-free environment in the medical and health system has been improved,but the smoking status is still not optimistic.The supervision mechanism should be improved to promote the tobacco control work.
