Vitapex糊剂治疗根尖周炎伴牙周炎患者的临床研究

Clinical trial of Vitapex paste in the treatment of patients with periapical periodontitis and periodontitis

目的观察Vitapex糊剂治疗根尖周炎伴牙周炎患者的临床效果。方法将84例患者依据随机数字分为对照组42例与试验组42例。对照组使用无菌棉签+碘酚作为牙周填充材料,每周1次,根管填充。试验组使用Vitapex糊剂作为牙周填充材料,每周1次,根管填充。每次根管填充完成后,将盐酸米诺环素软膏注入牙周袋中,以患者感觉局部有肿胀感及部分溢出为准。每次治疗后2 h内禁食禁水、禁止漱口,每周接受1次治疗,持续4周。比较2组的疼痛情况指标、牙周指标、牙龈沟液炎性细胞因子指标、治疗效果以及药物不良反应发生情况。结果治疗后,试验组和对照组的总有效率分别为95.24%(40例/42例)和78.57%(33例/42例),差异有统计学意义(P<0.05)。治疗后,牙菌斑指数(PLI)分别为1.42±0.38和1.75±0.40,牙周袋深度(PD)分别为(3.22±0.54)和(4.10±0.68)mm,牙龈沟液白细胞介素-6(IL-6)分别为(1.90±0.71)和(3.24±0.82)ng·m L^(-1),牙龈沟液C反应蛋白(CRP)分别为(6.05±0.62)和(6.77±0.72)ng·m L^(-1),差异均有统计学意义(均P<0.05)。试验组和对照组的药物不良反应发生率分别为9.52%(4例/42例)和7.14%(3例/42例),差异无统计学意义(P>0.05)。结论根尖周炎伴牙周炎患者用Vitapex糊剂进行根管填充,有助于改善牙周病变的多项指标,降低疼痛程度,提升治疗效果且安全性可靠。

Objective To observe the clinical effect of Vitapex paste in the treatment of periapical periodontitis with periodontitis.Methods The 84 patients were divided into control group(42 cases)and treatment group(42 cases)based on random numbers.Control group was given sterile cotton swabs+iodophenol as periodontal filling materials,once a week,root canal filling.Treatment group was given Vitapex paste as periodontal filling material,once a week,root canal filling.After each root canal filling is completed,minocycline hydrochloride ointment was injected into the periodontal bag,based on the patients’feeling of local swelling and partial overflow.Fasting,water and gargling were forbidden within 2 hours after each treatment.Patients received treatment once a week for 4 weeks.The pain indicators,periodontal indicators,gingival crevicular fluid inflammatory cytokine indicators,treatment effects and adverse drug reactions of the two groups were compared.Results After treatment,the total effective rates of treatment group and control group were 95.24%(40 cases/42 cases)and78.57%(33 cases/42 cases),with significant difference(P<0.05).After treatment,the plaque index(PLI)were1.42±0.38 and 1.75±0.40,the periodontal pocket depth(PD)were(3.22±0.54)and(4.10±0.68)mm,the gingival crevicular fluid interleukin-6(IL-6)were(1.90±0.71)and(3.24±0.82)ng·m L^(-1),gingival crevicular fluid C-reactive protein(CRP)were(6.05±0.62)and(6.77±0.72)ng·m L^(-1),the difference were all statistically significant(all P<0.05).The incidence of adverse drug reactions in treatment group and control group were 9.52%(4 cases/42 cases)and 7.14%(3 cases/42 cases),without significant difference(P>0.05).Conclusion For patients with periapical periodontitis and periodontitis who use root canal therapy,the use of Vitapex paste for root canal filling can help to improve many indicators of periodontal disease,reduce pain,improve treatment effect and reliable safety.
