摘要
目的探讨心脏瓣膜置换术在患者术前访视中采用微信平台互动模式干预的临床应用价值。方法以随机数字表法,将2019年5月至2020年9月80例接受心脏瓣膜置换术治疗的患者分为观察组和对照组,每组40例。对照组实施常规术前访视;观察组在此基础上,实施基于微信平台的互动模式干预。采用t检验比较两组患者干预前后负性情绪变化,入室前以及麻醉诱导前的血流动力学变化;采用χ^(2)检验比较两组手术配合度及患者护理满意度情况。结果干预前,两组抑郁自评量表(SDS)、焦虑自评量表(SAS)评分比较差异无统计学意义(t值分别为0.404、0.251,P值均>0.05);干预后,观察组SDS、SAS评分明显低于对照组,差异有统计学意义(t值分别为5.939、6.134,P值均<0.05);入室前,两组舒张压(DBP)、心率(HR)、收缩压(SBP)比较差异无统计学意义(P值均>0.05);麻醉诱导前,观察组上述指标水平均明显低于对照组,差异有统计学意义(t值分别为2.564、2.899、2.546,P值均<0.05);观察组患者手术配合优良率为100.0%,明显高于对照组90.0%,差异有统计学意义(χ^(2)=4.211,P<0.05);观察组患者护理满意率为97.5%,显著高于对照组85.0%,差异有统计学意义(χ^(2)=3.914,P<0.05)。结论采用微信平台的互动模式干预对心脏瓣膜置换术患者术前访视具有积极的临床价值,不仅有效地改善患者情绪,减少血流动力学波动,还能够提高患者手术配合度及护理满意度,值得推广。
Objective To explore the application effect of interactive intervention based on the WeChat platform in preoperative visits for patients undergoing heart valve replacement.Methods According to the random number table method,80 patients undergoing heart valve replacement from May 2019 to September 2020 were divided into the control group(40 cases)and the observation group(40 cases).The control group performed routine preoperative visits,and the observation group implemented interactive intervention based on the WeChat platform based on the control group.The changes of negative emotion before and after intervention and the hemodynamics changes before entering the operating room and before anesthesia induction were observed using t test.Patients'compliance with surgery and the nursing satisfaction of the two groups were compared using the χ^(2) test.Results Before the intervention,the two groups had no significant difference(t=0.404,0.251;all P>0.05),and the difference was not statistically significant in the scores of the self-rating depression scale(SDS)and self-rating anxiety scale(SAS).After the intervention,compared with the control group at the same period,the above scores in the observation group were significantly lower(t=5.939,6.134;all P<0.05),and the difference was statistically significant.Before surgery,the diastolic blood pressure(DBP),heart rate(HR),and systolic blood pressure(SBP)between the two groups were compared,and there is no significant difference(P>0.05),and the difference was no statistically significant.Before anesthetic induction,compared with the control group at the same period,the levels of the above indicators in the observation group were significantly lower(P<0.05),and the difference was statistically significant(t=2.564,2.899,2.546;all P<0.05).The excellent and good rate of patients'compliance with surgery in the observation group was 100%,significantly higher than the control group,which is 90%,and the difference was statistically significant(χ^(2)=4.211,P<0.05).The nursing satisfaction rate in the observation group was 97.5%,which is significantly higher than the 85.0%in the control group,and the difference was statistically significant(χ^(2)=3.914,P<0.05).Conclusions The application of interactive intervention based on the WeChat platform in the preoperative visit for patients undergoing heart valve replacement can significantly improve the emotional state,reduce hemodynamic fluctuations,and improve patients´compliance with surgery and nursing satisfaction.Therefore,the interactive intervention based on the WeChat platform in the preoperative visit,is worth of spreading and applying.
