摘要
目的调查北京市某区家庭医生签约服务居民认知度、签约率、满意度与中医服务需求,为进一步完善基层医疗卫生管理提供依据。方法采用分层-整群随机抽样方法,抽取城区15个社区参加基本公共卫生服务的18岁及以上居民以调查问卷形式进行电话调查,调查不同群组对家庭医生居民认知度、签约率、满意度与中医服务需求。结果实际外呼18558份电话,其中接通-成功访问为4186份,占比22.56%。接受调查患者中77.4%的居民表示知道该区开展的家庭医生签约服务,19.9%的居民表示不知道,2.8%的居民表示记不清。从居民类型来看,计划生育人群、糖尿病人群、脑卒中人群等对家庭医生签约服务的认知度较高,χ^(2)检验显示,差异有统计学意义(P<0.01)。66.0%的居民表示签约了家庭医生服务,26.5%的居民表示没有签,7.5%的居民表示记不清。计划生育人群、糖尿病人群、脑卒中人群等对家庭医生签约服务的签约率较高,χ^(2)检验显示,差异有统计学意义(P<0.01)。签约居民对家庭医生签约服务的满意度得分为(86.65±15.88)分。从居民类型来看,孕产妇、残疾人、低保等人群对家庭医生签约服务的满意度评价较高,而脑卒中、65岁及以上老年人等人群对家庭医生签约服务的满意度评价相对较低,单因素方差分析显示,差异无统计学意义(P>0.05)。受访居民80.24%表示有中医服务需求,需求内容主要体现在中医药适宜技术服务及健康指导等方面。结论被纳入管理的社区居民对基层卫生服务机构卫生服务满意度尚可,认知度、签约率有待进一步提升,社区居民对家庭医生中医技术服务的需求度较高。
Objective To investigate the cognition,contracting rate,satisfaction rate and TCM service demand of residents who contracted family doctors in a certain district of Beijing to provide a basis for further improving primary medical and health management.Methods Stratified cluster random sampling method was used to select residents aged 18 and above who participated in basic public health services in 15 urban communities to conduct a telephone survey in the form of questionnaire.The awareness,contract rate and satisfaction of residents of different groups to family doctors were investigated.Results There were 18558 actual calls,among which 4186 were connected and successfully visited,accounting for22.56%.Among the surveyed patients,77.4%of residents knew about the family doctor contract service,19.9%said they did not know,and 2.8%said they did not remember.From the perspective of the type of residents,the family planning population,the diabetes population,and the stroke population have a higher awareness of the service of family doctors.Theχ^(2)test shows that the difference is statistically significant(P<0.01).66.0%of residents said they had signed up for family doctor services,26.5%of residents had not signed,and 7.5%of residents said they could not remember.The family planning population,diabetes population,stroke population,etc.had a higher rate of signing family doctor services.Theχ^(2)test showed that the difference was statistically significant(P<0.01).The satisfaction score of the contracted residents with the contracted services of family doctors was(86.65±15.88)points.From the perspective of the types of residents,pregnant women,disabled persons,and subsistence allowances have higher satisfaction ratings for family doctors’contracted services,while strokes,people aged 65 and above have relatively higher satisfaction ratings for family doctors’contracted services lower,one-way analysis of variance showed that the difference was not statistically significant(P>0.05).80.24%of the residents interviewed indicated that they have a demand for Chinese medicine services,and the content of the demand is mainly reflected in the appropriate technical services and health guidance of Chinese medicine.Conclusion The community residents who were included in the management have good satisfaction with the health services of the grassroots health service institutions.However,their awareness and signing rate need to be further improved.The community residents have a high demand for the technical services of family doctors in traditional Chinese medicine.
