肝动脉造影联合超声互补式诊断对肝细胞肝癌的诊断价值分析

Analysis on the value of complementarity diagnosis of hepatic artery angiography combined with ultrasound for hepatocellular carcinoma

目的:分析肝动脉造影联合超声互补式诊断对肝细胞肝癌的诊断价值。方法:选取医院肿瘤科收治的116例肝细胞肝癌患者,所有患者均接受肝动脉造影、肝脏超声造影及肝动脉造影联合超声造影互补式诊断,均以肝组织病理活检结果为准,分析其诊断的阳性率、灵敏度、特异度及准确率。结果:肝动脉造影、肝脏超声造影及二者联合互补检查的阳性率分别为74.07%、69.52%和90.70%;肝动脉造影、肝脏超声造影和二者联合诊断的灵敏度分别为80.00%、86.30%和97.44%,差异有统计学意义(x^(2)=3.832,x^(2)=4.768,x^(2)=8.158;P<0.05);肝动脉造影、肝脏超声造影和二者联合诊断的特异度分别为50.00%、68.75%和75.00%,差异有统计学意义(x^(2)=5.180,x^(2)=4.920,x^(2)=6.180;P<0.05);肝动脉造影、肝脏超声造影和二者联合诊断的准确率分别为72.22%、80.95%和95.35%,差异有统计学意义(x^(2)=2.782,x^(2)=4.566,x^(2)=7.976,P<0.05))。结论:肝动脉造影联合超声互补式诊断有助于提高临床肝细胞肝癌的诊断正确率,可降低临床误诊率和漏诊率。

Objective:To analyze the value of complementarity diagnosis of hepatic angiography combined with ultrasound in the diagnosis of hepatocellular carcinoma.Methods:116 patients with hepatocellular carcinoma who admitted to hospital were selected,and all of patients received hepatic artery angiography,hepatic ultrasound contrast and complementarity diagnosis of hepatic angiography combined with ultrasound contrast.And the results of the biopsy of hepatic tissue were used as standard to analyze the diagnostically positive rate,sensitivity,specificity and accuracy of different method.Results:The positive rates of hepatic artery angiography,hepatic ultrasound contrast and the complementarity examination of the two methods were 74.07%,69.52%,and 90.70%,respectively.The sensitivities of hepatic artery angiography,hepatic ultrasound contrast and the combined diagnosis were 80.00%,86.30%and 97.44%(x^(2)=3.832,x^(2)=4.768,x^(2)=8.158,P<0.05),respectively.The specificities of hepatic artery angiography,hepatic ultrasound contrast and the combined diagnosis of the two methods were 50.00%,68.75%and 75.00%(x^(2)=5.180,x^(2)=4.920,x^(2)=6.180,P<0.05),respectively.And the accuracies of hepatic artery angiography,hepatic ultrasound contrast and the combined diagnosis of the two methods were 72.22%,80.95%and 95.35%(x^(2)=2.782,x^(2)=4.566,x^(2)=7.976,P<0.05),respectively.Conclusions:The complementary diagnosis of hepatic artery angiography combined with ultrasound contrast would contribute to increased diagnostic accuracy on clinically hepatocellular carcinoma,and can reduce clinically misdiagnosis rate and the rate of missed diagnosis.
