参苓胃消胶囊联合XELOX方案治疗大肠癌的临床研究

Clinical study on Shenlingweixiao capsule combined with XELOX regimen in the treatment of colorectal cancer

目的观察参苓胃消胶囊联合XELOX方案对大肠癌术后脾气亏虚型患者细胞免疫功能及生活质量的影响。方法将2019年5月—2020年4月河北省沧州中西医结合医院收治的80例大肠癌术后脾气亏虚型患者随机分为2组,对照组40例术后给予XELOX方案化疗,研究组40例在化疗基础上加服参苓胃消胶囊,连续治疗12周。观察2组治疗后中医症状积分、生活质量评分、血清肿瘤标记物[癌胚抗原(CEA)、糖类抗原199(CA199)]、细胞免疫功能[T淋巴细胞亚群CD3+、CD4+、CD8+、CD4+/CD8+及自然杀伤细胞(NK)]、肿瘤血管生成指标[血管内皮生长因子(VEGF)、转化生长因子β1(TGF-β1)、基质金属蛋白酶-9(MMP-9)]、炎症反应指标[C反应蛋白(CRP)]变化情况及不良反应发生情况。结果治疗后2组中医症状积分及血清CEA、CA199、CD8+、VEGF、TGF-β1、MMP-9、CRP水平均明显下降(P均<0.05),且研究组治疗后上述指标均明显低于对照组(P均<0.05)。治疗后2组生活质量评分及CD3+、CD4+、CD4+/CD8+、NK细胞均明显上升(P均<0.05),且研究组以上指标均明显高于对照组(P均<0.05)。研究组和对照组不良反应发生率分别为10.0%(4/40)、32.5%(13/40),研究组明显低于对照组(P<0.05)。结论参苓胃消胶囊联合XELOX方案能够改善大肠癌术后脾气亏虚型患者症状,抑制患者肿瘤血管生成,减轻全身炎症反应,改善机体免疫功能,减少不良反应发生,提高患者生活质量。

Objective It is to observe the effect of Shenlingweixiao capsule combined with XELOX regimen on cellular immune function and quality of life in patients with spleen-qi deficiency after colorectal cancer surgery.Methods Eighty patients with postoperative spleen-qi deficiency of colorectal cancer admitted to Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from May 2019 to April 2020 were randomly divided into 2 groups.40 patients in the control group were given XELOX chemotherapy after surgery,and 40 patients in the study group were treated with Shenlingweixiao capsules on the basis of chemotherapy,both groups were treated continuously for 12 weeks.The changes in the scores of TCM symptom and quality of life,serum tumor markers[carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199)],cellular immune function[T lymphocyte subsets CD3+,CD4+,CD8+,CD4+/CD8+and natural killer cells(NK)],tumor angiogenesis indicators[vascular endothelial growth factor(VEGF),transforming growth factorβ1(TGF-β1),matrix metalloproteinase-9(MMP-9)],inflammatory response index[C-reactive protein(CRP)]after treatment and occurrence of adverse reactions were observed in the two groups.Results After treatment,the TCM symptom scores and serum levels of CEA,CA199,CD8+,VEGF,TGF-β1,MMP-9,and CRP in the two groups were significantly decreased(all P<0.05),and the above indicators in the study group were significantly lower than those in the control group after treatment(all P<0.05).After treatment,the quality of life scores,CD3+,CD4+,CD4+/CD8+,and NK cells in the two groups were increased significantly(all P<0.05),and the above indicators in the study group were significantly higher than those in the control group(all P<0.05).The incidence of adverse reactions in the study group and the control group were 10.0%(4/40)and 32.5%(13/40),respectively,and the study group was significantly lower than the control group(P<0.05).Conclusion Shenlingweixiao capsule combined with XELOX regimen can improve the symptoms of patients with spleen-qi deficiency after colorectal cancer surgery,inhibit tumor angiogenesis,reduce systemic inflammation,improve immune function,reduce adverse reactions,and improve their quality of life.
