Comparison of nasal catheter high flow oxygen inhalation and non-invasive positive pressure ventilation in the treatment of neonatal acute respiratory failure

摘要 目的:分析经鼻导管高流量吸氧(HFNC)与无创正压通气(CPAP方式、NIPPV方式)治疗新生儿急性呼吸衰竭的疗效。方法:2019年1月-2020年12月收治新生儿急性呼吸衰竭患儿60例,随机分为A组(HFNC)、B组(CPAP)、C组(NIPPV),各20例。比较三组治疗前后的血气指标水平、氧合指数(OI)及并发症发生率。结果:A组动脉血氧分压(PaO2)与OI水平均明显高于B组和C组,差异有统计学意义(P<0.05)。A组并发症发生率低于B组和C组,差异有统计学意义(P<0.05)。结论:HFNC用于新生儿急性呼吸衰竭的效果理想,能够改善血气指标水平与氧合指数,降低并发症发生率。 Objective:To analyze the efficacy of nasal catheter high flow oxygen inhalation(HFNC)and non-invasive positive pressure ventilation(CPAP,NIPPV)in the treatment of neonatal acute respiratory failure.Methods:From January 2019 to December2020,60 newborns with acute respiratory failure were selected,they were randomly divided into the three groups:the group A(HFNC),the group B(CPAP),and the group C(NIPPV) with 20 cases in each group.The blood gas index,oxygenation index(OI)and the incidence of complications were compared among the three groups before and after treatment.Results:The levels of arterial oxygen partial pressure(PaO2)and OI in the group A were significantly higher than those in the group B and the group C,the difference was statistically significant(P<0.05).The incidence of complications in the group A was significantly lower than that in the group B and the group C,the difference was statistically significant(P<0.05).Conclusion:HFNC is effective in the treatment of neonatal acute respiratory failure,which can improve blood gas index and oxygenation index and reduce the incidence of complications.
