沙库巴曲缬沙坦对心衰患者预后和炎症因子的影响

The effect of sacubitril/valsartan on prognosis and inflammatory factors in patients with heart failure

目的:探讨沙库巴曲缬沙坦对慢性心衰患者预后及对炎症因子(IL-6、TNF-α)的影响。方法:2019年6月-2020年10月收治慢性心衰患者60例,随机分为两组,各30例。两组均给予常规抗心衰治疗,对照组给予缬沙坦治疗,观察组给予沙库巴曲缬沙坦治疗。治疗3个月后,比较两组患者NT-proBNP、IL-6、TNF-α及心脏彩超等相关指标水平。结果:观察组治疗3个月后NT-proBNP、IL-6、TNF-α、左室舒张末期内径(LVEDD)、左心房内径(LAD)水平较治疗前均降低,左室射血分数(LVEF)较治疗前增加,差异有统计学意义(P<0.05);对照组NT-proBNP水平较治疗前降低,LVEF较治疗前增加,差异有统计学意义(P<0.05)。观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论:沙库巴曲缬沙坦对心衰患者的预后及降低炎症因子的效果优于缬沙坦,可推广使用。

Objective:To explore the effects of sacubitril/valsartan on the prognosis and inflammatory factors(IL-6,TNF-α)in patients with chronic heart failure.Methods:From June 2019 to October 2020,60 cases of patients with chronic heart failure were enrolled,they were randomly divided into two groups with 30 cases in each group.Both groups were treated with conventional anti-heart failure therapy.The control group was treated with valsartan.The observation group was treated with sacubitril/valsartan.After three months of treatment,the levels of NT-proBNP,IL-6,TNF-αand cardiac color Doppler ultrasound were compared between the two groups.Results:The levels of NT-proBNP,IL-6,TNF-α,left ventricular end-diastolic diameter(LVEDD)and left atrial diameter(LAD)in the observation group were lower than those before treatment,and left ventricular ejection fraction(LVEF)was higher than before treatment,the differences were statistically significant(P<0.05).NT-proBNP level in the control group was lower than before treatment,left ventricular ejection fraction(LVEF)was higher than before treatment,the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The effect of sacubitril/valsartan on the prognosis of patients with heart failure and reducing inflammatory factors is better than valsartan,which can be popularized.
