他汀类药物对急性心肌梗死PCI患者CIN的影响 被引量:1

摘要 目的:探讨他汀类药物减少急性心肌梗死患者对比剂肾病(CIN)发生率的作用。方法:选取2016年1月-2018年12月经皮冠状动脉介入(PCI)治疗的急性心肌梗死患者962例,随机分为两组,各481例。研究组PCI术前口服阿托伐他汀60~80 mg;对照组术前未服用阿托伐他汀;两组患者术后均常规口服双联抗血小板和阿托伐他汀10~20 mg/d治疗。比较两组患者生化指标水平和CIN发生率。结果:研究组术后3 d尿素氮(BUN)、血肌酐(Scr)、血清胱抑素C(CysC)水平均低于对照组,肌酐清除率(Ccr)水平高于对照组,差异有统计学意义(P<0.05);研究组CIN发生率低于对照组,差异有统计学意义(P<0.05)。结论:急性心肌梗死患者PCI治疗期间应用他汀类药物,可缓解肾功能损伤,减少CIN发生。 Objective:To investigate the effect of statins in reducing the occurrence of contrast nephropathy(CIN)in patients with acute myocardial infarction.Methods:From January 2016 to December 2018,962 cases of acute myocardial infarction patients with PCI were enrolled,they were randomly divided into two groups with 481 cases in each group.The study group took 60-80 mg of atorvastatin before PCI.The control group did not take atorvastatin before PCI.Both groups were treated with oral dual antiplatelet and 10-20 mg/d of atorvastatin after surgery.The biochemical indexes and the incidence of CIN between the two groups were compared.Results:After 3 days of operation,the levels of BUN,Scr and CysC in the study group were lower than those in the control group,and the Ccr level was higher than that in the control group,the differences were statistically significant(P<0.05).The incidence of CIN in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The application of statins for acute myocardial infarction patients with PCI can relieve renal function injury and reduce the occurrence of CIN.
