家庭医生工作室模式对社区膝骨关节病患者中医药服务效能的影响

Influence of family doctor studio mode on traditional Chinese medicine service efficiency for patients with knee osteoarthritis in the community

目的:探索家庭医生工作室模式对社区膝关节病(KOA)患者中医药服务效能的影响。方法:于2018年8月通过分层抽样对200例KOA患者进行问卷调查,其中男性83例,女性117例。年龄40~89岁。比较中医服务融入家庭医生工作室工作前(2017年)和后(2018年)的中医药服务效能。结果:与2017年比较,2018年中医家庭医生预约率增长了40.0%,履约率增长了19.44%;平均等候时间由(29.60±9.92)min减少至(10.12±3.08)min,差异均有统计学意义(P<0.05)。目标患者群接受中医适宜技术诊疗及其标化工作量的增长率为48%。结论:通过分时段预约就诊等手段可明显减少患者中医适宜技术就诊的等候时间,提高预约履约率,提高就诊满意度,提高医疗服务的效率,有助于形成患者接受疾病诊疗与医生提供医疗服务之间的良性循环。家庭医生工作室以标化工作量为基准,可将西医、中医、公共卫生、护理等不同类别的工作做客观比较,标化工作量可体现家庭医生工作室的部分效能。

Objective:To explore the influence of the family doctor studio mode on the efficiency of traditional Chinese medicine(TCM)services for patients with knee osteoarthropathy(KOA)in the community.Methods:In August 2018,a questionnaire survey of 200 KOA patients was conducted through stratified sampling,including 83 males and 117 females.Their ages were from 40 to 89 years old.The efficiency of traditional Chinese medicine services before(2017)and after(2018)when traditional Chinese medicine services were integrated into the family doctor studio were compared.Results:Compared with 2017,the appointment rate of TCM family doctors in 2018 increased by 40.0%,and the compliance rate increased by 19.44%;the average waiting time was reduced from(29.60±9.92)min to(10.12±3.08)min,and the difference was statistically significant(P<0.05).The growth rate of the workload of the target group of patients receiving TCM appropriate technology diagnosis and treatment and standardization was 48%.Conclusion:It can significantly reduce the waiting time of patients with TCM appropriate technology,improve the performance rate of appointment,improve the satisfaction of patients,and improve the efficiency of medical services by appointment in different periods,which helps to form a virtuous cycle between patients receiving disease diagnosis and treatment and doctors providing medical services.Based on standardized workload,family doctor studio can objectively compare different types of work,such as western medicine,traditional Chinese medicine,public health,nursing and so on,and the standardized workload can reflect part of the efficiency of family doctor studio.
