地屈孕酮与米非司酮对围绝经期功能失调性子宫出血患者的影响

Influence of Dydrogesterone and Mifepristone in Patients with Perimenopausal Dysfunctional Uterine Bleeding

目的:探究地屈孕酮与米非司酮治疗对围绝经期功能失调性子宫出血患者的影响。方法:选取本院2018年5月-2019年5月收治的86例围绝经期功能失调性子宫出血患者展开对照研究。根据随机数字表法分为观察组(地屈孕酮联合米非司酮治疗)与对照组(米非司酮治疗),各43例。比较两组月经量、子宫内膜厚度、血红蛋白水平及性激素水平。结果:与对照组总有效率(74.42%)相比,观察组(93.02%)显著较高(P<0.05)。用药前两组子宫内膜厚度、月经量、血红蛋白水平比较差异无统计学意义(P>0.05);用药后,观察组子宫内膜厚度、月经量显著低于对照组(P<0.05),观察组血红蛋白水平显著高于对照组(P<0.05)。用药前两组各项性激素指标比较差异无统计学意义(P>0.05);用药后,观察组促黄体生成素(LH)、促卵泡生成素(FSH)、孕酮(P)、雌二醇(E2)水平均低于对照组(P<0.05)。结论:对围绝经期功能失调性子宫出血患者采取地屈孕酮联合米非司酮治疗后,可显著降低月经量、子宫内膜厚度,改善血红蛋白、性激素水平,此方法值得应用与推广。

Objective:To investigate the influence of Dydrogesterone and mifepristone in patients with perimenopausal dysfunctional uterine bleeding.Method:A total of 86 patients with perimenopausal dysfunctional uterine bleeding admitted to our hospital from May 2018 to May 2019 were selected to conduct a control study.According to random number table,patients were divided into the observation group (Dydrogesterone and Mifepristone treatment) and the control group (Mifepristone treatment),43 cases in each group.The menstrual blood volume,endometrial thickness,levels of hemoglobin and gonadal hormone of the two groups were compared.Result:The total effective rate in the observation group (93.02%) was significantly higher than 74.42% of the control group (P<0.05).Before medication,there were no significant differences in the endometrial thickness,menstrual blood volume and hemoglobin level between the two groups (P>0.05);after medication,the endometrial thickness and menstrual blood volume in the observation group were lower than those in the control group (P<0.05);the hemoglobin level in the observation group was significantly higher than that of the control group (P<0.05).There were no significant differences in the gonadal hormone indexes between the two groups before treatment (P<0.05);after treatment,the luteinizing hormone (LH),follicle stimulating hormone (FSH),progesterone (P) and estradiol (E2) levels in the observation group were lower than those in the control group (P<0.05).Conclusion:Dydrogesterone combined with Mifepristone can remarkably reduce the menstrual blood volume and endometrial thickness,and improve the levels of hemoglobin and gonadal hormone for patients with perimenopausal dysfunctional uterine bleeding.It is worthy of clinical application and promotion.
