摘要
目的:探析在新生儿窒息病情评价中实施脐血动脉血气分析联合Apgar评分的临床价值。方法:从2020年1-10月择取4 426例单胎足月新生儿,将其中83例窒息新生儿纳入研究组,其中阴道分娩53例,均为轻度窒息(早产4例,足月49例);剖宫产30例,26例轻度窒息(早产13例,足月13例),4例重度窒息(早产2例,足月2例),并根据窒息程度分为两组,将轻度窒息新生儿纳入研究一组,将重度窒息新生儿纳入研究二组,其他正常新生儿纳入对照组,所选研究对象均在分娩后即刻实施Apgar评分,并抽取脐动脉血进行血气分析,对评估及检测结果进行对照分析。结果:研究一组和研究二组Apgar评分均比对照组低,且研究二组评分低于研究一组(P<0.05);研究一组和研究二组脐血动脉pH值均比对照组低,且研究二组低于研究一组(P<0.05);研究一组和研究二组二氧化碳分压均比对照组高,且研究二组高于研究一组(P<0.05);研究一组和研究二组氧分压水平均比对照组低,且研究二组低于研究一组(P<0.05);研究二组脐动脉酸中毒发生率高于研究一组(P<0.05)。结论:在新生儿窒息病情评价中实施脐血动脉血气分析联合Apgar评分的临床价值显著。
Objective:To explore the clinical value of umbilical artery blood gas analysis combined with Apgar score in the assessment of neonatal asphyxia.Method:From January to October 2020,4 426 singleton full-term newborns were selected and 83 cases of asphyxia were included in the study group,including 53 cases of vaginal delivery,all of which were mild asphyxia (4 cases of premature delivery,49 cases of full-term);30 cases of cesarean section,26 cases of mild asphyxia (13 cases of premature delivery,13 cases of full-term),4 cases of severe asphyxia (2 cases of premature delivery,2 cases of full-term).The newborns with mild asphyxia were included in the study group 1,the severe asphyxia newborns were included in the study group 2,and other normal newborns were included in the control group.All the selected subjects were given Apgar score immediately after delivery,and the umbilical artery blood gas analysis was conducted,and the evaluation and detection results were compared and analyzed.Result:According to the analysis of Apgar score,the score of study group 1 and study group 2 were lower than that of control group,and the score of study group 2 was lower than that of study group 1 (P<0.05).According to the pH value analysis of umbilical cord blood artery,both study group 1 and study group 2 were lower than the control group,and study group 2 were lower than study group 1 (P<0.05).From the analysis of PCO2,both study group 1 and study group 2 were higher than the control group,and study group 2 was higher than study group 1 (P<0.05).From the analysis of oxygen partial pressure level,both study group 1 and study group 2 were lower than the control group,and study group 2 was lower than study group 1 (P<0.05).The incidence of umbilical artery acidosis was higher in group 2 than in group 1 (P<0.05).Conclusion:The clinical value of umbilical cord blood arterial blood gas analysis combined with Apgar score in the evaluation of neonatal asphyxia is significant.
