阴道彩色多普勒超声诊断子宫内膜癌的符合率及与肿瘤标志物的相关性

Coincidence Rate of Vaginal Color Doppler Ultrasonography in Diagnosis of Endometrial Cancer and Its Correlation with Tumor Markers

目的:探讨阴道彩色多普勒超声诊断子宫内膜癌的符合率及与肿瘤标志物的相关性。方法:选择2018年3月-2020年1月子宫内膜癌患者62例,设为观察组;选择同期治疗的子宫内膜增生患者58例,设为对照组。两组均给予阴道超声检查;采用化学发光法测定两组CA125、CA199、CA153水平;采用Pearson相关性分析软件对阴道彩色多普勒超声诊断子宫内膜癌的符合率与肿瘤标志物进行相关性分析。结果:62例子宫内膜癌患者经阴道彩色多普勒超声检查确诊58例,诊断符合率为93.55%。子宫内膜癌声像图表现为内膜明显增厚,且不对称,呈不规则高、中、弱回声或混合型回声,且内部回声分布杂乱不均,且病灶部位具有丰富血流信号;而子宫内膜增生患者超声下可见内膜对侧性增厚,内膜回声分布均匀,与肌层分解清楚,未见明显异常血流信号;观察组肿瘤标志物CA125、CA199、CA153水平均高于对照组(P<0.05);Pearson相关性分析结果表明,子宫内膜癌患者阴道彩色多普勒超声诊断子宫内膜癌的符合率与肿瘤标志物CA125、CA199、CA153水平呈正相关性(P<0.05)。结论:阴道彩色多普勒超声用于子宫内膜癌患者中能获得较高的符合率,且与肿瘤标志物结果存在相关性。

Objective:To investigate the coincidence rate of vaginal color Doppler ultrasound in diagnosis of endometrial cancer and its correlation with tumor markers.Method:A total of 62 patients with endometrial cancer from March 2018 to January 2020 were selected as the observation group,and 58 patients with endometrial hyperplasia treated in the same period were selected as the control group.The levels of CA125,CA199 and CA153 in the two groups were measured by chemiluminescence method;the correlation between the coincidence rate in the diagnosis of endometrial cancer by vaginal color Doppler ultrasound and tumor markers was analyzed by Pearson correlation analysis software.Result:Among 62 cases of endometrial cancer,58 cases were diagnosed by transvaginal color Doppler ultrasound,and the diagnostic coincidence rate was 93.55%.The sonogram of endometrial carcinoma showed that the endometrium was obviously thickened and asymmetric,with irregular high,medium,weak echo or mixed echo,and the internal echo distribution was disorderly and uneven,and the lesion site had rich blood flow signals;however,in patients with endometrial hyperplasia,contralateral endometrial thickening was observed under ultrasound,with uniform endometrial echo distribution and clear muscular decomposition,and no obvious abnormal blood flow signal was observed.The levels of CA125,CA199 and CA153 of tumor markers in the observation group were higher than those in the control group (P<0.05);the results of SPSS Pearson correlation analysis showed that the coincidence rate in the diagnosis of endometrial cancer by vaginal color Doppler ultrasound was positively correlated with the levels of tumor markers of CA125,CA199 and CA153 (P<0.05).Conclusion:Vaginal color Doppler ultrasound in patients with endometrial cancer can obtain a high coincidence rate,and there is a correlation with the results of tumor markers.
