腹腔镜手术干预在子宫内膜异位症患者中的疗效观察及对凝血功能的影响

Effect of Laparoscopic Surgery on Endometriosis Patients and Its Effect on Coagulation Function

目的:探讨腹腔镜手术干预在子宫内膜异位症患者中的疗效观察及对凝血功能的影响。方法:选取笔者所在医院2017年2月-2019年2月收治的87例子宫内膜异位症患者为研究对象,采用随机数字法分为两组,对照组34例,研究组53例,对照组应用传统开腹手术治疗,研究组应用腹腔镜手术治疗,对比两组治疗后凝血功能指标、免疫功能指标及临床疗效。结果:术毕及术后3 d,研究组PT、APTT、TT均短于对照组,PLT均高于对照组,差异均有统计学意义(P<0.05),术毕及术后3 d两组D-D比较差异无统计学意义(P>0.05);术后3 d研究组IgM、IgA、IgG均高于对照组,差异均有统计学意义(P<0.05);研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05);研究组手术、肠胃功能恢复、住院时间均短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05)。结论:在子宫内膜异位症的治疗中,应用腹腔镜手术进行治疗取得了满意的效果,其能够降低对机体免疫功能及凝血功能的影响,进而可促进病灶周围损伤组织的修复,临床疗效更高,值得进一步推广应用。

Objective:To investigate the therapeutic effect of laparoscopic intervention in patients with endometriosis and its influence on coagulation function.Method:A total of 87 patients with endometriosis admitted to our hospital from February 2017 to February 2019 were selected as study subjects.The patients were randomly divided into two groups by numerical method,including 34 patients in the control group and 53 cases in the study group,the control group was treated with traditional open surgery,while the study group was treated with laparoscopic surgery.The effects of different treatment methods on coagulation function,immune function and clinical efficacy of the two groups were compared.Result:After surgery and 3 d after surgery,PT,APTT and TT in the study group were shorter than those in the control group,while PLT in the study group was higher than that in the control group,the differences were statistically significant (P<0.05),but there was no statistically significant difference in D-D between the two groups after surgery and 3 d after surgery (P>0.05).Three days after operation,IgM,IgA and IgG in the study group were higher than those in the control group,the differences were statistically significant (P<0.05).The total effective rate in the study group was higher than that in the control group,the difference was statistically significant (P<0.05).The operation,gastrointestinal function recovery and hospitalization time in the study group were shorter than those in the control group,and the amount of intraoperative blood loss was lower than that in the control group,the differences were statistically significant (P<0.05).Conclusion:In the treatment of endometriosis,the application of laparoscopic surgery has achieved satisfactory results,which can reduce the impact on the body’s immune function and coagulation function,and then promote the repair of damaged tissues around the lesion,with higher clinical efficacy,and is worthy of further promotion and application.
