高危妊娠患者APTT、TT、FIB与D-二聚体的联合诊断效果

Combination Diagnostic Effect of APTT,TT,FIB and D-dimer Levels in High-risk Pregnancy Patients

目的:探究高危妊娠患者APTT、TT、FIB与D-二聚体的联合诊断效果。方法:选取2019年2月-2020年11月笔者所在医院收治的高危妊娠患者96例作为研究组,选取同时期到笔者所在医院进行健康体检的正常妊娠者96例作为对照组,均接受APTT、TT、FIB与D-二聚体检测。对比两组APTT、TT、FIB与D-二聚体,以及不同指标检测的敏感性、特异性和准确性。结果:与正常妊娠组比较,高危妊娠组的APTT、TT指标更低(P<0.05),FIB与D-二聚体等凝血水平均更高(P<0.05);与APTT、TT、FIB及D-二聚体单一检测比较,四项联合检测阳性例数最高;与TT、FIB与D-二聚体单一检测比较,APTT检测的敏感性、准确性更高(P<0.05),与APTT、TT、FIB单一检测比较,D-二聚体检测特异性更高(P<0.05),与APTT、TT、FIB、D-二聚体单一检测比较,四项联合检验的敏感性、特异性和准确性更高(P<0.05)。结论:高危妊娠患者接受APTT、TT、FIB与D-二聚体联合检验可以提高其临床诊断准确率,有效反映孕妇体内的凝血、纤溶功能情况,具有一定的诊断价值,值得推广应用。

Objective:To explore the combined diagnostic effect of APTT,TT,FIB and D-dimer levels in high-risk pregnancy patients.Method:A total of 96 high-risk pregnancy patients admitted to our hospital from February 2019 to November 2020 were selected as the study group,and 96 normal pregnancy patients admitted to the hospital of the author during the same period for physical examination were selected as the control group.All pregnant women were tested for APTT,TT,FIB and D-dimer.The levels of APTT,TT,FIB and D-dimer were compared between the two groups,and the sensitivity,specificity and accuracy of different indicators were detected.Result:Compared with normal pregnancy group,the indexes of APTT and TT in high-risk pregnancy group were lower(P<0.05),and the levels of FIB and D-dimer were higher on average(P<0.05).Compared with single detection of APTT,TT,FIB and D-dimer indicators,the number of positive cases in four combined tests was the highest;compared with TT,FIB and D-dimer,APTT showed higher sensitivity and accuracy(P<0.05);compared with APTT,TT and D-FIB,D-dimer showed higher specificity(P<0.05).The sensitivity,specificity and accuracy of four combined tests were higher than those of APTT,TT,FIB and D-dimer alone(P<0.05).Conclusion:The combination test of APTT,TT,FIB and D-dimer in high-risk pregnancy patients can improve the clinical diagnostic accuracy and positive detection rate,and it can effectively reflect the coagulation and fibrinolysis function in pregnant women,which has certain diagnostic value and is worthy of promotion and application.
