CT影像学检查在肺结核合并肺癌诊断中的价值分析

The Value Analysis of CT Imaging Examination in the Diagnosis of Pulmonary Tuberculosis Complicated with Lung Cancer

目的:分析CT影像学检查在肺结核合并肺癌诊断中的应用价值。方法:回顾性分析湖北省荆州市胸科医院2016年7月-2019年10月住院或门诊治疗的73例肺结核合并肺癌患者病例资料,将其作为观察组,同期选择75例单纯性肺结核患者作为对照组。所有患者均予以胸部CT影像学扫描检查,比较两组患者临床症状、体征及胸部CT影像学特征。结果:两组患者咳嗽、咯血、胸闷、气短、胸痛、消瘦、盗汗、发热及淋巴结肿大等临床症状、体征比较差异无统计学意义(P>0.05);观察组患者片状影、分叶征、团块影、棘刺征、毛刺征及空泡征等CT影像学特征比例均高于对照组,而条索影、空洞征、斑点影、钙化等CT影像学特征比例均低于对照组,差异均有统计学意义(P<0.05)。结论:肺结核合并肺癌存在CT影像学特征,CT影像学检查有助于鉴别诊断肺结核合并肺癌与单纯性肺结核,具有重要的诊断价值。

Objective:To analyze the value of CT imaging examination in the diagnosis of pulmonary tuberculosis complicated with lung cancer.Method:The case data of 73 patients with pulmonary tuberculosis complicated with lung cancer who hospitalized or outpatients in the Chest Hospital of Jingzhou in Hubei from July 2016 to October 2019 were retrospective analyzed,they were selected as the observation group,and 75 patients with simple pulmonary tuberculosis were selected as the control group during the same period.All patients were given chest CT imaging examination,and the clinical symptoms and signs of two groups were compared.The chest CT imaging characteristics of two groups were compared.Result:There were no statistically significant differences between the two groups in clinical symptoms and signs such as cough,hemoptysis,chest tightness,shortness of breath,chest pain,emaciation,night sweats,fever and lymph node enlargement(P>0.05).The proportion of CT imaging features such as flaky sign,foliation sign,mass sign,thorn sign,burr sign and vacuole sign in the observation group were higher than those in the control group,while the proportion of CT imaging features such as strip shadow,hollow sign,speckle shadow,calcification in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:There are some CT imaging features in pulmonary tuberculosis complicated with lung cancer.CT imaging examination is helpful for the differential diagnosis of pulmonary tuberculosis complicated with lung cancer and simple pulmonary tuberculosis,and it has an important diagnostic value.
