不同透析方式治疗尿毒症患者的临床效果

Clinical Effect of Different Dialysis Methods in the Treatment of Patients with Uremia

目的:探讨在治疗尿毒症患者时采取不同的透析方式对其皮肤瘙痒、血清甲状腺素、β_(2)微球蛋白水平的影响。方法:选取2017年1月-2020年6月在笔者所在医院就诊的尿毒症患者124例。患者入院后给予不同的透析方案,按照透析方式分为两组。对照组(n=62)采取常规血液透析滤过治疗,研究组(n=62)实施高通量血液透析治疗,对比治疗效果。结果:研究组皮肤瘙痒程度评分(2.12±0.41)分,明显低于对照组的(3.66±0.55)分(t=17.676 2,P<0.05);研究组治疗后血清甲状旁腺素(433.53±84.68)mmol/L,低于对照组的(512.44±90.63)mmol/L,β_(2)微球蛋白水平(34.21±8.67)mg/L,低于对照组的(44.72±9.61)mg/L(t=5.009 4、6.393 8,P<0.05);研究组治疗有效率95.2%,显著高于对照组的83.9%(χ^(2)=4.210 7,P<0.05)。结论:对尿毒症患者采取高通量血液透析治疗效果更佳,有助于缓解其皮肤瘙痒症状,改善血清甲状旁腺素与β_(2)微球蛋白水平,建议临床应用。

Objective:To investigate the effects of different dialysis methods on skin pruritus,serum thyroxine and β_(2)-microglobulin levels in uremic patients.Method:A total of 124 uremic patients in our hospital from January 2017 to June 2020 were selected.Patients were given different dialysis programs after admission,and they were divided into two groups according to the dialysis mode.The control group(n=62)was treated with conventional hemodialysis filtration,the study group(n=62)was treated with high flux hemodialysis,and the treatment effect was compared.Result:The pruritus score of the study group was(2.12±0.41)points,which was significantly lower than(3.66±0.55)points of the control group(t=17.6762,P<0.05);the serum parathyroid hormone was(433.53±84.68)mmol/L of the study group,which was lower than(512.44±90.63)mmol/L of the control group,and the level of β_(2)-microglobulin of(34.21±8.67)mg/L was lower than(44.72±9.61)mg/L of the control group(t=5.0094,6.3938,P<0.05).The effective rate of the study group was 95.2%,significantly higher than 83.9%of the control group(χ^(2)=4.2107,P<0.05).Conclusion:The effect of high flux hemodialysis on uremic patients is better,which is helpful to relieve the skin itching symptoms and improve the serum parathyroid hormone and β_(2)-microglobulin levels.It is suggested to be used in clinic.
