冠心病患者血清Lpa、Hcy水平与病情严重程度的相关性分析

Analysis of Correlation between the Levels of Serum Lpa and Hcy and the Severity of Disease in Patients with Coronary Heart Disease

目的:研究冠心病(coronary heart disease,CHD)患者血清脂蛋白a(Lpa)、同型半胱氨酸(Hcy)水平与病情严重程度的相关性。方法:将2020年2-10月笔者所在医院心内科收治的69例冠心病患者设为研究组,将同期入院体检的75例健康受试者设为对照组。比较两组血清Lpa和Hcy水平。比较研究组不同病情严重程度患者血清Lpa、Hcy水平。分析血清Lpa、Hcy检测在CHD临床诊断中的应用效能;分析CHD患者血清Lpa、Hcy水平与病情严重程度的相关性。结果:研究组血清Lpa、Hcy水平均显著高于对照组,差异有统计学意义(P<0.05);ROC曲线分析显示,血清Lpa、Hcy水平对CHD均具有较高诊断价值,其Cut-off值分别为214.67 mg/L、16.91μmol/L;重度组血清Lpa、Hcy水平均高于中度组及轻度组,且中度组均高于轻度组,差异有统计学意义(P<0.05);Spearman秩相关系数显示,CHD患者血清Lpa、Hcy水平与病情严重程度呈正相关(r=0.732、0.872,P<0.05)。结论:CHD患者血清Lpa、Hcy水平与病情严重程度呈正相关,可有效评估CHD病情情况。

Objective:To study the correlation between the levels of serum lipoprotein a(Lpa)and homocysteine(Hcy)and the severity of disease in patients with coronary heart disease(CHD).Method:A total of 69 patients with CHD admitted in the department of cardiology of our hospital from February to October 2020 were selected as the study group,and 75 healthy subjects admitted in our hospital for physical examination during the same period were selected as the control group.The levels of Lpa and Hcy were compared between the two groups.Serum Lpa and Hcy levels of patients with different severity of disease in the study group were compared.The application efficacy of serum Lpa and Hcy in the clinical diagnosis of CHD was analyzed.The correlation between serum Lpa,Hcy and severity of CHD patients was analyzed.Result:The serum Lpa and Hcy levels in the study group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).ROC curve analysis showed that serum Lpa and Hcy levels had high diagnostic value for CHD,with Cut-off values of 214.67 mg/L and 16.91μmol/L,respectively.Serum Lpa and Hcy levels in the severe group were higher than those in the moderate group and the mild group,and those in the moderate group were higher than those in the mild group,the differences were statistically significant(P<0.05).Spearman rank correlation coefficient showed that serum Lpa and Hcy levels were positively correlated with the severity of CHD(r=0.732,0.872,P<0.05).Conclusion:Serum Lpa and Hcy levels in CHD patients are positively correlated with the severity of disease,which can effectively evaluate the condition of CHD.
