风险管理模式对基层医院行腹腔镜阑尾切除术患者围手术期心理应激和并发症的影响

Effect of risk management model on perioperative psychological stress and complications of laparoscopic appendectomy in primary hospitals

目的探讨和分析风险管理模式对基层医院行腹腔镜阑尾切除术的患者围手术期心理应激和并发症的影响。方法选取暨南大学附属顺德医院2019年1月2020年6月期间收治的行腹腔镜阑尾切除术患者102例。采用随机数字表法分人对照组和观察组,每组51例。对照组患者实施常规护理干预模式,观察组患者联合上级医院实施风险管理模式干预。比较两组干预前后焦虑、抑郁心理应激状态,以及并发症发生率和生活质量等。结果干预后,观察组心理应激评分均显著低于对照组(P值均<0.05)。观察组总并发症发生率显著低于对照组(P<0.05)。干预后,观察组肌体疼痛低于对照组,总体健康和躯体角色评分均显著高于对照组(P值均<0.05)。结论基层医院联合上级医院实施风险管理模式干预,可降低行腹腔镜阑尾切除术患者的心理应激程度和并发症发生风险,同时减轻对患者生活质量的影响。

Objective To explore and analyze the effect of risk management model on perioperative psychological stress and complications of laparoscopic appendectomy in primary hospital.M ethods 102 patients undergoing laparoscopic appendectomy in Shunde Hospital Affiliated to Jinan University from January 2019 to June 2020 were selected and divided into two groups by the random number table method.51 patients in the control group were given routine nursing intervention mode,and 51 patients in the observation group were given risk management mode intervention cooperated with the superior hospital.The psychological stress such as anxiety and depression,complication rate and quality of life before and after the intervention were compared between the two groups.Results After intervention,the psychological stress score of the observation group were significantly lower than those of the control group(all P<0.05).The total incidence of complication in the observation group was significantly lower than that in the control group(P<0.05).After the intervention,the scores of body pain were lower than those in the control group,general health and physical role in the observation group were higher than those in the control group(all P<0.05).Conclusion The implementation of risk management mode intervention by primary hospitals in conjunction with superior hospitals can reduce the psychological stress and risk of complications in patients undergoing laparoscopic appendectomy,and reduce the impact on the quality of life of patients at the same time.
