摘要
目的:分析清热解毒方联合布地奈德治疗重症肺炎的效果及对肺表面活性蛋白、应激因子水平的影响。方法:此次研究的对象为116例重症肺炎患者,按治疗方式的不同将其分为单一组(予以布地奈德治疗)和联合组(予以清热解毒方联合布地奈德治疗),每组58例。比较两组治疗前后的血清肺表面活性蛋白(Surfactant Protein,SP)水平和降钙素原(Procalcitonin,PCT)、血管紧张素Ⅰ(AngiotensinⅠ,AngⅠ)、基质金属蛋白酶2(MMP2)、血管紧张素Ⅱ(AngiotensinⅡ,AngⅡ)水平,并统计两组的有创呼吸机、无创呼吸机、总呼吸机的应用时间。结果:治疗之前,两组的SP、PCT、AngⅠ、MMP2、AngⅡ水平差异无统计学意义(P>0.05)。治疗之后,联合组的SP、PCT、AngⅠ、MMP2、AngⅡ水平均低于单一组(P<0.05);联合组有创呼吸机、无创呼吸机、总呼吸机的应用时间均短于单一组(P<0.05)。结论:将清热解毒方结合布地奈德应用于重症肺炎患者的治疗中,不但能有效缩短呼吸机的实际应用时间,还可显著降低患者体内的SP水平,减少应激反应,治疗效果理想。
Objective:To analyze the effects of the Qingre Jiedu decoction(清热解毒方)plus budesonide on the levels of lung surfactant protein and stress factors in severe pneumonia patients.Methods:116 cases were divided into the single group(took budesonide)and the combined group(took the Qingre Jiedu decoction plus budesonide)according to treatment methods,58 cases in each.Results:Before treatment,there was no statistical difference in the levels of SP,PCT,AngⅠ,MMP2,and AngⅡbetween groups(P>0.05).After treatment,the above indicators in the combined group were lower than those in the single group(P<0.05).The application time of invasive ventilator,non-invasive ventilator,and total ventilator in the combined group was all shorter than the single group(P<0.05).Conclusion:Treating severe pneumonia with the Qingre Jiedu decoction plus budesonide can not only effectively shorten the actual application time of the ventilator,but also significantly reduce the level of pulmonary surfactant protein in the patient’s body,and reduce stress response,with ideal effects.
