中医综合外治法治疗压疮的疗效观察

The effects of TCM comprehensive external therapy on pressure ulcer

目的:探讨中医综合外治法治疗压疮的临床疗效。方法:将2016年3月-2020年2月越秀区中医医院收治的压疮患者86例随机分为对照组和观察组,对照组给予负压引流术治疗,观察组在负压引流术的基础上联合使用中医综合外治法治疗,疗程为21 d。比较两组的临床疗效、溃疡愈合率、上皮组织生长时间、肉芽组织生长时间、创口愈合时间、治疗前后创面症状评分、创面面积、视觉模拟疼痛评分(Visual Analogue Scale,VAS),检测患者治疗前后血清白细胞介素(Interleukin,IL)-1、IL-6、超敏C-反应蛋白(Hypersensitive C-reactive Protein,hs-CRP)的水平。结果:观察组的总有效率和溃疡愈合率显著高于对照组(P<0.05);观察组的上皮组织生长时间、肉芽组织生长时间、创口愈合时间显著短于对照组(P<0.05);两组的创面症状评分、创面面积、VAS评分均较治疗前显著降低(P<0.05),且观察组的创面症状评分、创面面积、VAS评分均低于对照组(P<0.05);两组血清hs-CRP、IL-1、IL-6水平均较治疗前显著降低(P<0.05),且观察组血清hs-CRP、IL-1、IL-6水平均低于对照组(P<0.05)。结论:中医综合外治法治疗压疮可以提高疗效,促进溃疡愈合,减轻疼痛,缩短愈合时间,抑制炎症反应。

Objective:To explore the clinical efficacy of TCM comprehensive external therapy on pressure ulcers.Methods:86 cases from March 2016 to February 2020 were randomly divided into the control group and the observation group.The control group was treated by negative pressure drainage.The observation group was treated by negative pressure drainage plus TCM comprehensive external therapy for 21 days.Results:After treatment,the total efficiency and ulcer healing in the treatment group were significantly higher than those in the control group(P<0.05);the epithelial tissue growth time,granulation tissue growth time,and wound healing time in the treatment group was significantly shorter than the control group(P<0.05).After treatment,the wound symptom scores,wound areas,and VAS scores in the groups were significantly lower than before treatment(P<0.05),and those indicators in the treatment group were lower than the control group(P<0.05).After treatment,the levels of serum hs-CRP,IL-1 and IL-6 in the groups were significantly lower than before treatment(P<0.05),and those indicators in the treatment group were lower than the control group(P<0.05).Conclusion:TCM comprehensive external therapy on pressure ulcers can improve the curative effects,promote ulcer healing,reduce pain,shorten the healing time,and inhibit the inflammatory responses.
