中医辨证治疗延缓慢性肾脏病进展的回顾性临床研究

A retrospective study on delaying the progression of chronic kidney disease by TCM differentiation and treatment

目的:观察中药汤剂辨证治疗对慢性肾脏病(Chronic Kidney Disease,CKD)3b-4期患者肾功能进展的影响。方法:回顾性分析2012年12月-2015年12月之间黑龙江省中医药科学院肾病科的CKD 3b-4期(血肌酐在177~442μmol/L)患者的临床资料,随机分为治疗组59例(基础治疗联合中药汤剂),对照组54例(基础治疗联合中成药)。计算两组治疗6个月、12个月后血肌酐均值、尿素氮均值、肾小球滤过率(Glomerular Filtration Rate,GFR)均值、24小时尿蛋白定量均值,以及记录两组患者进入透析时间。结果:治疗组比对照组的血肌酐、尿素氮升高地慢,24小时尿蛋白定量减少地多,GFR降低地慢,而且延长患者进入透析的时间。结论:中药汤剂辨证治疗可延缓CKD3b-4期患者的肾功能进展。

Objective:To observe the effects of TCM differentiation and treatment on renal function progression in CKD 3b-4 stage patients.Methods:The patients were divided into the treatment group of 59 cases(took basic treatment plus decoction)and the control group of 54 cases(received basic treatment plus TCM patent medicines).Results:The increase of creatinine and urea nitrogen in the treatment group were slower than the control group,24-hour urine protein quantitative reduction was much more,the decrease of glomerular filtration was slower,and the time interval of patients received dialysis was longer.Conclusion:TCM differentiation and treatment can delay renal function progression of CKD 3b-4 stage patients.
