腹部B超与阴道B超联用诊断妇产科急腹症的临床价值

Clinical value of abdominal B-ultrasound combined with vaginal B-ultrasound in the diagnosis of gynecological acute abdomen

目的探讨妇产科急腹症采用腹部B超联合阴道B超的诊断价值。方法152例疑似妇产科急腹症患者,采用SSA320型超声诊断仪进行诊断。分别进行腹部B超、阴道B超检查及联合检查,分析腹部B超与阴道B超联合诊断的价值。结果在所选取急腹症患者当中,经腹部B超检查,确诊115例(75.66%),漏诊6例(3.95%),误诊31例(20.39%);经阴道B超检查,确诊137例(90.13%),漏诊1例(0.66%),误诊14例(9.21%);腹部B超联合阴道B超检查确诊145例(95.39%),漏诊1例(0.66%),误诊6例(3.95%)。结论采用腹部B超与阴道B超相联合的方法来诊断妇产科急腹症,能提高诊断准确率,为临床诊治提供依据,因而可将其当作检查急腹症的首选方法;另外,B超检查经济、实用且方便,易在妇产科尤其是基层医院推广应用。

Objective To discuss the diagnostic value of abdominal B-ultrasound combined with vaginal B-ultrasound in the diagnosis of gynecological acute abdomen.Methods A total of 152 patients with suspected gynecological acute abdomen were diagnosed with SSA320 ultrasonic diagnostic apparatus.All patients underwent abdominal B-ultrasound,vaginal B-ultrasound and single and combined examination to analyze the value of combined diagnosis of abdominal B-ultrasound and vaginal B-ultrasound.Results 115 cases(75.66%)were confirmed by abdominal B-ultrasound,6 cases(3.95%)were missed,and 31 cases(20.39%)were misdiagnosed;137 cases(90.13%)were confirmed by vaginal B-ultrasound,1 case(0.66%)was missed,and 14 cases(9.21%)were misdiagnosed;145 cases(95.39%)were confirmed by abdominal B-ultrasound combined with vaginal B-ultrasound,1 case(0.66%)was missed,and 6 cases(3.95%)were misdiagnosed.Conclusion The combined examination of abdominal B-ultrasound and vaginal B-ultrasound to the diagnosis of gynecological acute abdomen can improve the diagnostic accuracy and provide a basis for clinical diagnosis and treatment.Therefore,it can be used as the first choice for examination of acute abdomen.In addition,B-ultrasound examination is economical,practical and convenient,and it is easy to popularize and apply in obstetrics and gynecology,especially in primary hospitals.
