以跨理论模型为指导的健康教育联合治疗性沟通在白癜风患者中的应用效果 被引量:16

Application effect of health education guided by trans-theoretical model combined with therapeutic communication in patients with vitiligo

摘要 目的探讨以跨理论模型为指导的健康教育联合治疗性沟通在白癜风患者中的应用效果。方法选取2017年2月—2020年6月郑州大学第一附属医院皮肤科门诊收治的白癜风患者120例,按简单随机数表法分为研究组和对照组,每组各60例。两组患者均进行常规治疗,对照组给予治疗性沟通,研究组给予跨理论模型为指导的健康教育联合治疗性沟通。干预前后采用症状自评量表(SCL-90)、Herth希望量表(HHI)、健康测量量表(SF-36)对两组患者进行调查。结果干预后,研究组患者SCL-90量表抑郁因子、焦虑因子得分均低于对照组,差异有统计学意义(P<0.05)。干预后,研究组患者HHI量表总分、SF-36量表各维度得分均高于对照组,差异均有统计学意义(P<0.05)。结论以跨理论模型为指导的健康教育联合治疗性沟通应用于白癜风患者护理干预能有效改善患者心理状态,提高其治疗依从性、希望水平和生活质量。 Objective To explore the nursing effect of health education guided by trans-theoretical model combined with therapeutic communication in patients with vitiligo.Methods A total of 120 patients with vitiligo who were admitted to Department of Dermatology in the First Affiliated Hospital of Zhengzhou University from February 2017 to June 2020 were selected and they were divided into the study group and the control group according to simple random number table method,with 60 cases in each group.Patients in both groups received conventional treatment.The control group was given therapeutic communication,while the study group was given trans-theoretical model-guided health education combined with therapeutic communication.Symptom Check List-90(SCL-90),Herth Hope Index(HHI)and The Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36)were used to investigate patients of the two groups before and after the intervention.Results After intervention,the depression factors and anxiety factors of the SCL-90 scale in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the intervention,the total score of the HHI scale and the score of each dimension of SF-36 scale of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusions Health education guided by trans-theoretical model combined with therapeutic communication can effectively improve the mental state of patients with vitiligo and improve their treatment compliance,hope level and quality of life.
