影响205例长期维持性腹膜透析患者生存预后的因素分析

Analysis of influencing factors for survival prognosis in 205 patients with continuous ambulatory peritoneal dialysis

目的:探讨影响205例长期维持性腹膜透析(PD)患者生存预后的因素。方法:回顾性分析205例长期维持性PD患者的病例资料,依据其预后进行分组,其中预后良好为预后良好组(n=148),预后不良的患者分为预后不良组(n=57),分析影响患者生存预后的危险因素。结果:年龄(≥70岁)、透析时间、GLU、CCI(≥5)、透析充分性(GFR、总Kt/V、残余肾Kt/V)、Hb、Alb、HDL、hs-CPR均是影响长期维持性PD患者生存预后的单因素;年龄(≥70岁)、透析时间、GLU、透析充分性、Hb、Alb及hs-CPR均是影响长期维持性PD患者生存预后的独立危险因素。结论:长期维持性PD患者生存预后不良的独立危险因素为年龄、原发病、透析时间、GLU、透析充分性、Hb、Alb及hs-CPR,临床需进行针对性、有效干预来提高患者的生存率。

Objective To investigate the factors affecting the survival prognosis of 205 patients with continuous ambulatory peritoneal dialysis(PD).Methods The case data of 205 patients with continuous ambulatory PD were retrospectively analyzed.The patients were grouped according to prognosis.Patients with good prognosis were included in good prognosis group(n=148),and patients with poor prognosis were included into poor prognosis(n=57).The risk factors affecting the survival prognosis of patients were analyzed.Results Age(≥70 years old),dialysis time,GLU,CCI(≥5),dialysis adequacy(GFR,total Kt/V,residual kidney Kt/V),Hb,Alb,HDL and hs-CPR were all single factors for survival prognosis of patients with continuous ambulatory PD.Age(≥70 years),dialysis time,GLU,dialysis adequacy,Hb,Alb and hs-CPR were independent risk factors for survival prognosis of patients with continuous ambulatory PD.Conclusion The independent risk factors for poor survival prognosis of patients with continuous ambulatory PD are age,primary disease,dialysis time,GLU,dialysis adequacy,Hb,Alb and hs-CPR.Clinically,targeted and effective intervention is needed to improve the survival rate of patients.
