低频电刺激对盆腔脏器脱垂盆底重建术后尿潴留的预防的影响 被引量:13

Preventive Effect of Low Frequency Electrical Stimulation on Urinary Retention after Pelvic Floor Reconstruction of Pelvic Organ Prolapse

摘要 目的:探讨低频电刺激对盆腔脏器脱垂盆底重建术后尿潴留的预防的影响。方法:选取我院2017年1月~2019年1月收治的盆腔脏器脱垂并需行盆底重建术患者78例,随机分为对照组、观察组各自39例。对照组患者盆底重建术后常规盆底肌康复训练,观察组患者盆底重建术后第1~5 d给予膀胱区、骶3区低频电刺激。比较两组患者第一次排尿情况、临床疗效、排尿情况、流尿率。结果:观察组尿潴留发生率2.50%显著低于对照组20.00%;观察组临床总有效率100.00%显著高于对照组84.62%;术后3d以及术后7d,对照组膀胱内压力低于观察组,残余尿量高于观察组;术后7d,观察组平均尿流率、排尿次数显著高于对照组,排尿时间显著短于对照组。结论:对盆腔脏器脱垂盆底重建术患者术后采用低频电刺激治疗,可有效提高尿流率,改善排尿情况,降低尿潴留发生率。 Objective To investigate the preventive effect of low frequency electrical stimulation on urinary retention after pelvic floor reconstruction of pelvic organ prolapse.Methods 78 patients with pelvic organ prolapse underwent pelvic floor reconstruction in our hospital from January 2017 to January 2019 were enrolled,and randomly divided into two groups,each with 39 cases.Control group was given the routine nursing,while the observation group received the low frequency electrical stimulation of third sacral nerve root and bladder from the 1^(st) day to 5^(th) day after operation.The first urination,clinical efficacy,urination and urine flow rate were compared between the two groups.Results The incidence of urinary retention in the observation group was 2.50%,which was significantly lower than 20.00% in the control group.The total clinical effective rate of the observation group was 100.00%,which was significantly higher than 84.62% of the control group.At the postoperative 3d and 7d,the intravesical pressure of the observation group was significantly higher than that of the control group,and the residual urine volume was significantly lower than that of the control group.At the postoperative 7d,the average urinary flow rate and urination frequency in the observation group were significantly higher than those in the control group,and the urination time was significantly shorter than that in the control group.Conclusion Low-frequency electrical stimulation for patients with pelvic organ prolapse underwent pelvic floor reconstruction can effectively improve the urine flow rate,and reduce the incidence of urinary retention.
