C-反应蛋白与前白蛋白比值、T淋巴细胞亚群在新生儿细菌感染性肺炎中的变化及其相关性 被引量:22

Changes of C-reactive protein to prealbumin ratio and T lymphocyte subsets in neonatal bacterial infection pneumonia and their correlation

摘要 目的:探讨血清C-反应蛋白(CRP)与前白蛋白(PA)比值、T淋巴细胞亚群在新生儿细菌感染性肺炎中的变化及其相关性。方法:选取2019年3月~2020年3月期间我院收治的500例新生儿细菌感染性肺炎患儿作为细菌感染组。选取同期收治的非细菌感染性肺炎患儿500例作为非细菌感染组,另选同期出生的健康新生儿500例作为对照组。比较各组间血清CRP、PA、CRP/PA比值及T淋巴细胞亚群,并进行Pearson相关性分析和Logistic回归分析。结果:细菌感染组血清CRP、CRP/PA比值显著高于非细菌感染组和对照组,血清PA水平显著低于非细菌感染组和对照组。细菌感染组和非细菌感染组CD3+、CD4+、CD4+/CD8+显著低于对照组,CD8+显著高于对照组,细菌感染组和非细菌感染组间CD3+、CD4+、CD8+、CD4+/CD8+无统计学差异。细菌感染重症肺炎组CRP、CRP/PA、CD8+显著高于细菌感染轻症肺炎组,PA、CD3+、CD4+、CD4+/CD8+显著低于细菌感染轻症肺炎组。经过Pearson相关性分析,细菌感染性肺炎新生儿CRP与CD3+、CD4+、CD4+/CD8+均呈负相关,与CD8+呈正相关;PA与CD3+、CD4+、CD4+/CD8+均呈正相关,与CD8+呈负相关;CRP/PA与CD3+、CD4+、CD4+/CD8+均呈负相关,与CD8+呈正相关。经过Logistic回归分析,CRP、PA、CRP/PA是细菌感染性肺炎新生儿的影响因素。结论:CRP、PA、CRP/PA比值在细菌感染性肺炎新生儿中表达异常,且与其病情严重程度和免疫抑制有关。 Objective To investigate the ratio of serum C-reactive protein(CRP)to prealbumin(PA)and T lymphocyte subsets in neonatal bacterial infection pneumonia.Methods 500 cases of neonatal bacterial infection pneumonia were selected as bacterial infection group from March 2019 to March 2020.Select 500 cases of non-bacterial infection pneumonia in the same period,In addition,500 healthy newborns were selected as control group.Serum CRP、PA、CRP/PA ratio and T lymphocyte subsets were compared between groups,and Pearson correlation analysis and Logistic regression analysis were carried out.Results The ratio of serum CRP,CRP/PA in bacterial infection group were significantly higher than that in non-bacterial infection group and control group,and the serum PA level was significantly lower than that in non-bacterial infection group and control group.The CD3,CD4,CD4/CD8 of bacterial infection group and non-bacterial infection group were significantly lower than that of control group,and the CD8 was significantly higher than that of control group.There was no significant difference between bacterial infection group and non-bacterial infection group.The CRP,CRP/PA,CD8+ of bacterial infection severe pneumonia group were significantly higher than that of bacterial infection mild pneumonia group,PA,CD3+,CD4+,CD4+/CD8+significantly lower than that of bacterial infection mild pneumonia group.Through Pearson correlation analysis,CRP of bacterial infection pneumonia neonatal were negatively correlated with CD3+,CD4+,CD4+/CD8+,positively correlated with CD8+;PA of bacterial infection pneumonia neonatal were positively correlated with CD3+,CD4+,CD4+/CD8+,negatively correlated with CD8+;CRP/PA of bacterial infection pneumonia neonatal were negatively correlated with CD3+,CD4+,CD4+/CD8+,positively correlated with CD8+.Through Logistic regression analysis,CRP、PA、CRP/PA is the influencing factor of bacterial infectious pneumonia neonates.Conclusion The expression of CRP,PA,CRP/PA ratio are abnormal in neonates with bacterial infectious pneumonia,and it is related to its severity and immunosuppression.
