硕通镜联合输尿管软镜与单纯输尿管软镜对复杂性肾结石患者疼痛程度及预后的影响

Effect of Ureteroscope combined with Shuo Tong Ureteroscopy and simple flexible Ureteroscope on the Degree of Pain and Prognosis in Patients with complex Renal Calculi

目的比较在复杂性肾结石患者治疗中硕通镜联合输尿管软镜与单纯输尿管软镜对疼痛程度及预后的影响。方法选取2019年5月至2020年12月在河南省人民医院接受治疗的120例复杂性肾结石患者,根据随机数字表法将患者分为观察组(n=66)与对照组(n=54)。对照组采用单纯输尿管软镜治疗,观察组采用硕通镜联合输尿管软镜治疗,比较两组结石清除率、临床指标、氧化应激指标及疼痛程度。结果观察组结石清除率显著高于对照组(P<0.05);观察组术中出血量显著少于对照组,手术时间、住院时间显著短于对照组(P<0.05);治疗后,两组超氧化物歧化酶(SOD)均明显降低,皮质醇(Cor)、血清丙二醛(MDA)均明显升高,但观察组SOD明显高于对照组,Cor、MDA明显低于对照组(P<0.05);治疗后,两组视觉模拟评分法(VAS)评分降低(P<0.05),且观察组评分显著低于对照组(P<0.05)。结论将硕通镜联合输尿管软镜应用于复杂性肾结石患者治疗中,有利于降低疼痛程度,提高结石清除率,改善患者预后,效果较为显著。

Objective To compare the effect of ureteroscope combined with Shuo Tong ureteroscopy and simple flexible ureteroscope on the degree of pain and prognosis in the treatment of complex renal calculi.Methods From May 2019 to December 2020,120 patients with complicated renal calculi were selected from Henan People’s Hospital and divided into observation group(N=66)and control group(N=54)according to random number table.The control group was treated with simple flexible ureteroscope,and the observation group was treated with ureteroscope combined with Shuo Tong ureteroscopy.the stone clearance rate,clinical index,oxidative stress index and pain degree between the two groups were compared.Results The stone clearance rate in the observation group was significantly higher than that in the control group(P<0.05),the amount of bleeding in the observation group was significantly less than that in the control group,and the operation time and hospitalization time were significantly shorter than that in the control group(P<0.05).After treatment,the SOD in both groups decreased significantly,COR and MDA were significantly increased.However,SOD in the observation group was significantly higher than that in the control group,the levels of COR and MDA were significantly lower than those in the control group(P<0.05).After treatment,the VAS of the two groups decreased(P<0.05),and the score of the observation group was significantly lower than that of the control group(P<0.05).Conclusion The application of ureteroscope combined with Shuo Tong ureteroscopy in the treatment of complex renal calculi can reduce the degree of pain,improve the stone clearance rate and improve the prognosis of patients,the effect is more significant.
