两种子宫压迫缝合术治疗难治性产后出血的临床比较研究

Clinical comparative study on two kinds of uterine compression sutures in the treatment of refractory postpartum bleeding

目的:比较两种子宫压迫缝合术治疗难治性产后出血的效果。方法:2018年8月-2020年8月收治难治性产后出血患者80例,随机分为两组,各40例。对照组采用“8”字间断缝合;观察组采用改良B-Lynch缝合。两组患者同时给予缩宫素、欣母沛等药物治疗。比较两组治疗效果。结果:观察组术后2 h、24 h出血量均少于对照组,输血量也少于对照组,差异有统计学意义(P<0.05);观察组产后恶露持续时间、止血时间、住院时间及月经复潮时间均短于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:改良B-Lynch缝合治疗难治性产后出血的效果更为理想。

Objective:To compare the effect of two kinds of uterine compression sutures in the treatment of refractory postpartum hemorrhage.Methods:From August 2018 to August 2020,80 cases of patients with refractory postpartum hemorrhage were enrolled,they were randomly divided into two groups with 40 cases in each group.The control group was sutured with"8"word intermittent.The observation group was sutured with modified B-Lynch.Both groups were treated with oxytocin and hemabate.The therapeutic effects between the two groups was compared.Results:The bleeding loss volume of 2 h,24 h after operation was less than that of the control group,the blood transfusion volume was less than that of the control group,the differences were statistically significant(P<0.05).The duration of postpartum lochia,the time of hemostasis,the length of hospital stays and the time of menstrual renewal in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Modified B-Lynch suture is more effective in treating refractory postpartum hemorrhage.
