经胸床旁超声心动图在急性胸痛诊断及鉴别诊断中的应用价值

Application value of transthoracic bedside echocardiography in diagnosis and differential diagnosis of acute chest pain

目的:分析经胸床旁超声心动图在急性胸痛诊断及鉴别诊断中的应用效果。方法:2020年1-9月收治急性胸痛患者190例,所有患者均接受胸床旁超声心动图检查和常规心电图检查,分析检查结果。结果:经胸床旁超声心动图检查对急性胸痛患者的诊断符合率与心电图检查比较,差异无统计学意义(P>0.05)。经胸床旁超声检查误诊为冠心病的误诊率明显低于心电图检查,差异有统计学意义(P<0.05)。结论:经胸床旁超声心动图能够对急性胸痛进行诊断,并对急性胸痛的病因进行鉴别诊断,尤其在肺栓塞与主动脉夹层等非冠脉病变心源性诊断中更具优势。

Objective:To analyze the effect of transthoracic bedside echocardiography in the diagnosis and differential diagnosis of acute chest pain.Methods:From January to September 2020,190 cases of patients with acute chest pain were enrolled,all patients had underwent transthoracic bedside echocardiography and routine electrocardiogram,and the results were analyzed.Results:The diagnostic coincidence rate of transthoracic bedside echocardiography in patients with acute chest pain was compared with that of ECG,the difference was not statistically significant(P>0.05).The rate of misdiagnosis of coronary heart disease by transthoracic bedside ultrasound was significantly lower than that of ECG,the difference was statistically significant(P<0.05).Conclusion:Transthoracic bedside echocardiography can diagnose acute chest pain and differential diagnose the cause of acute chest pain.It is especially advantageous in the diagnosis of non-coronary artery disease such as pulmonary embolism and aortic dissection,and has clinical application value.
